POC Ervin Spivey (202) 307-1944 D -- It is the intent of the Department
of Justice (DOJ), Justice Management Division, Procurement Services
Staff to acquire subscription services for CD-Rom Medical Library
Services to include Medicare/Medicaid Library, State Medicaid
Libraries, and medical code policies CPT-4 and ICD-9 for the years 1994
through 1996. The DOJ intends to award a firm fixed-price contract with
one base year and 4 option years for subscription services on an annual
basis. The medical subscription service will be utilized by United
States Attorneys throughout the 94 judicial districts as resources
during trial preparation for Health Care Fraud Abuse. The requirement
is for 94 Medicare/Medicaid Library copies, 22 State Medicaid Library
copies,and 114 CPT-4 and ICD-9 Codes for 1994 through 1996 copies.
Annual shipping and handling charges shall be inclusive in the cost of
proposals. This is a combined synopsis/solicitation for commercial
items prepared in accordance with the format in Subpart 12.6, as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation and proposals are
hereby requested. A written solicitation will not be issued. This
solicitation number DOJ-SAS-8-0011 is issued as a request for quotation
(RFQ). All references to "offers" are understood to be "quotations".
This solicitation document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 90-6. Contract
Line items (CLIN) are as follows: CLIN 0001, Medical/medicaid Library,
CLIN 0002, State Medicaid Library, CLIN 0003, CPT-4 and ICD-9 Codes
for 1994-1996, and CLIN 0004 Shipping and Handling Charges. A
distribution List of shipping addresses will be provided at the time of
contract award. FAR 52.121-1, Instructions to Offerors-commercial
Items, is incorporated by reference and applies to this solicitation.
The government will award a contract resulting from this
solicitation/request for quotation to the responsible offeror whose
quotation conforms to the solicitation and is most advantageous to the
Government, price and other factors considered. Offers shall include
a completed copy of the provision of FAR 52.212-3, Offeror
Representations and Certifications -- Commerical Items, with its
written quotation. The following clauses are incorporated by reference:
