synopsis/solicitation for Commercial items prepared in accordance with
the format in subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. The Request for Quote (RFQ) is F6550197QP296. The
provisions and clauses incorporated are those in Federal Acquisition
Circular 90-46, and it has a standard industrial code (SIC) of 3861.
Line Item 0001: Preventive Maintenance for CT Scanner System (Index
#7436): QTY 10, Unit of Issue Monthly, price per month with extended
total. Line Item 0002: Furnish & Install x-ray: QTY 10, Unit of Issue
Monthly, price per month with extended total. Line Item 0003: Emergency
Service Calls beyond the normal duty hours & days specified in the
Performance Work Statement, Div. 1, Para 3. Hourly rates are applied as
follows: 1. Mon-Sat (except Holidays) 5:00pm -- 8:00am. 2. Sunday and
Holidays. WageDetermination No.: 94-2017 Rev # 8 Applies. The
Contractor shall provide all, labor, materials, equipment,
transportation, tools for inspecting, reporting, servicing and all
preventive maintenance inspections and repairs necessary to keep the CT
system functioning in a safe reliable condition. Contractor shall
comply with applicable codes, manufacture's standards and
recommendations, and the requirements stated in the technical
provisions. Performance Period: 1 Oct 97- 31 Jul 97. Repair Service
Hours: Normal duty hours from 0730-1630 hrs, Monday- Friday, except
holidays observed at the Government installation. In event of a system
malfunction the contractor will be notified by either the Contracting
Officer or designated representative. Contractor must respond by phone
within 2 hrs of notification of system malfunction to designated part.
Contractor shall service system within 4 working hours of notification
time. At contract commencement, the contractor will provide Contracting
Officer with names and numbers of persons responsible for service.
Point of Contact: Contractor shall provide a primary and alternate
point of contact. Qualifications: Contractor shall have: a. Prior
experience maintaining CT scanner systems. b. training certificates to
ensure reliability, technical capability, engineering and technical
support. c. maintenance performance plan showing compliance with
requirements of Performance Work Statement, Division 2, Paragraph 2.
Permits: Contractor shall secure and pay for all required permits,
fees, and licenses. Work Location: Contractor shall report to Bldg.
24-800, Medical Equipment Repair (MAC), (RM. G195) during normal duty
hours prior to commencing work for each maintenance visit. Reports:
Contractor shall furnish documentation, including photographs,
radiation dosage measurement, and all applicable x-ray file exposures,
of all measurements and calibrations to ensure the system is
performing according to manufacturer's specifications. Upon completion,
the contractor shall submit an inspection report to the MERC Center or
designated representative to include; contract number, name of service
engineer, repairs completed or services accomplished, and signature and
date from government representative. Fire Safety Requirements:
Contractor shall comply with all Federal, State and Base fire and
safety regulations. Testing & Inspections: Perform all inspections,
test parts and systems specified herein, as stated in applicable codes
and standards. Contractor shall provide all necessary equipment,
material, labor necessary to properly perform the above. All testing
and inspection shall be observed by the Contracting Officer or
authorized representative. The following provisions apply 52.212-1,

any addenda 52.212-3, Advising Offerors to include a completed copy of
representation and certifications-commercial items with offer,
52-212-4, Contract terms and Conditions-Commercial items, applies to

Commercial items, applies to this acquisition. This is a unrestricked
