(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation, F11623-97-T3684, is issued as an request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 90-46. (iv) This is a total small business set-aside. The associated standard industrial classification code is 7991 and small business size standard $3.5 million. (v) There is one line item number and quantity is 12 months (MOs) (vi) Description of requirements for the items to be acquired. Services, Non-Personal: Contractor shall provide all labor, tools, equipment, materials, personnel, and transportation required conduct a aerobics program at Scott AFB, IL. Requirements and work include: (a) Instructor shall be CPR and First Aid certified and have an Aerobic Certification, i.e. IDEA, ACE. (b) Technical Requirements: Step classes, Interval classes(cardio plus strength), Slide classes, and Aerobic dance classes(low impact, Latin style). (c) Periods of performance: 24 classes per week, Class times may vary IAW customer demand, Classes will not be conducted on Federal holidays, down days or base closure because of weather, Suggested times are: 0600-0700, 1030-1130, 1130-1230, 1730-1830 Monday (M) through Friday (F) and 0900-1000 M, Wednesday (W), F, and Saturday (SA). Classes should include the following: Warm up, Aerobic segment with heart rate checks, cool down, and stretching. The Instructor should be able to educate class members on safety precautions and effective exercise principals(i.e., target heart rate). (d) Government will furnished the following equipment: Steps, Dumbbells, Rubber bands, Rubber tubing with handles, Aerobic Slides, and Stereo system with head microphone. Except for items listed as government furnished equipment, contractor is responsible for providing all materials necessary to conduct aerobics classes as per these specifications. (vii) Period of performance is 1 Oct 97 through 30 Sep 97. (viii) The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. Note: The government does not intend to make multiple awards (ix) The FAR provision 52.212-2, Evaluation -- Commercial Items, applied and quotes shall be evaluated for fair and reasonable price, technical acceptability, adherence to applicable clauses and provisions, and satisfactory performance record. (x) A completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items, must be submitted with quote (copies will be provided upon request). (xi) The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. Note: Offerors shallalso submit standard commercial warranties or guarantees for parts, labor, equipment, and/or installation. (xii) The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, for Paragraph (b) the following clauses apply: (6), (7), (8), and (9), and for Paragraph (c): (1), (2), and (5) also apply. (xiii) In accordance with the Service Contract Act, Wage Determination No. 94-2309 Revision No. 6 dated 12/12/1995 is hereby incorporated into this solicitation. (xiv) The Defense Priorities and Allocations System (DPAS) rating is DO-S10. (xv) The Commerce Business Daily numbered Note 1, 100% small business set-aside, applies.
