Z -- EMERGENCY ASBESTOS REMOVAL SOL 541-38-98 DUE 091597 POC Jeanelle Wright, (216) 838-6091 This is a combined synopsis/solicitation for commercial services prepared in accordance to the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation #541-38-98 is issued as a Request For Quotations (RFQ) under commercial services This solicitation document incorporates all applicable provisions and clauses in effect through Federal Acquisition Circular 90-46. This procurement is 100 % set aside for small business. The applicable SIC Code is 3292 and the size standard is $750. Quotations must reach the Contracting Officer, no later than September 15, 1997 @ 4:30 p.m. The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. 52.212-2 applies and quotations will be evaluated on price, past performance and location of facilities to provide emergency services within a timely manner. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items applies to this solicitation. 52.212-3, Offerors Representation and Certification -- Commercial Items must be submitted with offer. The Contracting Officer can provide copies of the form. The VA will award to the company offering the best overall value to the Government. Offerors shall submit pricing on decontamination area (each); Poly enclosure (SF); Background and Sample Clearance (each); Removal of top layer of asbestos floor tile and base including mastic (SF); Removal of additional layers of floor tile and base including mastic (SF); Removal of carpeting (SY); Removal of Pipe insulation for pipes less than 3" in diameter by glovebag (LF); Removal of pipe insulation 3"-8" in diameter, inclusive by glovebag (LF); Removal of pipe insulation for pipes less than 3" diameter by bulk (LF); Removal of pipe insulation 3"-8" diameter, inclusive by bulk (LF); Remove pipe insulation greater that 8" diameter by bulk (LF); Remove asbestos and 3" layer of contaminated soil (SF); Remove HVAC duct insulation (SF); Remove insulation from tanks, boilers and breeches (SF); Remove pressed asbestos board, siding, shield, ceiling insulation, paper insulation (SF); Remove sheetrock including joint compound and tape. (SF) Remove mechanical flex ducts, dampers and expansion joints (SF). Pricing shall be submitted for Base Year and two (2) one (1) year option renewals. Base Year: October 1, 1997 through September 30, 1998; Option Year 1: October 1, 1998 through September 30, 1999; Option Year 2: October 1, 1999 through September 30, 2000. Contractor shall provide VAMC with emergency paging numbers for a maximum two (2) hours response to any page, provide VAMC a maximum response time of 24 hours when emergency asbestos removal is required, provide certified asbestos workers experienced in emergency removal techniques, provide all necessary air monitoring, testing and documentation associated with asbestos removal. Contractor to remove all asbestos from the facility to an approved disposal site and maintain documentation of disposal site and location of material being removed from facility. It is estimated that approximately 320 man hours will be used every 12 months. Contractor will be required to work at both facilities when called upon. Contractor to maintain open permits for asbestos removal to allow for expedited response times. Contractor shall be responsible for damage to equipment and /or systems that result from the removal of asbestos materials. All work shall be in strict accordance with all applicable Federal, State and local regulations, standards and codes governing asbestos abatement and any other trade work accomplished in conjunction with the abatement.
