This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitues the only solicitation; proposals are being requested and a written solicitation will not be issued. II. This solicitation number MDA220-97-R-0010 is issued as a request for proposal (RFP). III. Base period Fy 1998 (1 October 97 through 30 September 98); Option Year 1 FY 1999; Option Year 2 FY 2000. The proposed acquisition has a requirement for corporate members to a full service physical fitness facility to be utilized for civilian and military personnel assigned to the DFAS Indianapolis Center. The required available services shall include: Qualified instuctor for formal health programs and services, indoor gymnasium, workout room with cardiovasular equipment, stationary and free weights, indoor swimming pool, running track, shower and secured locker facilities. This facility shall be available for use from 6:00AM until 8:00PM Monday through Friday. The base period of performance is for 12 months with two 12 months options. The facility must be within 1 mile of the DFAS Center Located at 8899 East 56th Street. VIII. FAR 52.212-1, Instructions to Offers-Commercial Items, is incorporated by reference and applies to this acquisition. Paragraph (a) of FAR 52.212-2 carries the following addendum: The following factors shall be used to evaluate offers: (1) Past Performance/Technical and (2) Price. Past Performance/Technical is more important than price. Award shall be made on a best value basis. X.
