Shepherd, 901-577-7228 FAX 901-577-7242 This is a combined synopsis/solicitation Notice of Intent to procure a Brand Name or Equal Jedmed House InfraRed/Video ENG System, for a commerical item prepared in accordance with the format in FAR Sub Part 12.6, as supplemented with additional information included in this notice. This announcement constitutes a formal Request for Quotation (RFQ 614-95-97), and a written solicitation WILL NOT be issued. The solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 90-46 and VAAR 96-28. The SIC is 3845. Prices shall be submitted as follows for each item: 1. Brand Name or Equal Jedmed Instrument Company Part Number (39-9060) to include House InfraRed/Video ENG System, complete, to include the IR/Video Goggle Assembly and Ocular Motor Module, House Eye Movement Computer with 1.44 MB Disk Drive and 420 MB Hard Drive, VGA Color Monitor, HP DeskJet Printer and complete Software. Offers shall be submitted FOB Destination, Contractor shall install equipment. Contractor shall be required to take as a trade-in equipment now in use and shall be required to remove all trade-in equipment. Contractor shall provide at NO COSTto the Government 2 user's/operators manuals and 2 service manuals with schematics and parts list. Item shall be installed and delivered within 30 days after award. The provisions at FAR 52.212-1 Instructions to Offerors -- Commerical Items, applies to this solicitation. All offerors are to include, with their offers a completed copy of provision 52.212-3 Offeror Representations and Certifications -- Commerical Items ( a copy of this provision may be requested by FAX to 901-577-7242. The following FAR clauses apply to this acquisition: FAR 52.212-4 Contract Terms and Conditions Required to implement Statutes or Executive Orders -- Commerical Items; as well as FAR Clauses cited in FAR 52.212-5; 52.222-26; 52.222-35; 52.222-36; 52.222-37; 52.225-3; 52.225-18; VAAR 852.219-70 is added by addendum. Contractor's who wish to be considered shall submit their proposals on Company Letterhead along with commerical literature and brochures of item being offered that meets the Governments requirements.
