AMENDMENT NO. 1 to Solicitation No. 583-112-97, which was originally
published on 8/18/97 (W-225 SN109967) as a combined
synopsis/solicitation for HOSPITAL LAUNDRY/LINEN SERVICES in accordance
with FAR Subpart 12.6 (commercial items). The solicitation is hereby
AMENDED as follows: 1. THE DUE DATE FOR RECEIPT OF PROPOSALS IS
EXTENDED. THE NEW PROPOSAL DUE DATE IS CLOSE OF BUSINESS FRIDAY,
SEPTEMBER 12, 1997 AT 4:30 P.M. (local time). 2. The quantity stated in
FAR 52.216-19, paragraph (b)(2) is REVISED to read "50,000 pounds". The
following requirements and information are hereby incorporated into
this solicitation: 1. All offerors shall submit with their offer a
Quality Assurance Plan/Program (QAP) detailing the procedures to be
used to ensure compliance with contract requirements. The QAP will be
used to evaluate an offeror's technical capability to meet the
requirements of the contract. 2. The turnaround time for services is
twenty-four (24) hours. 3. All services shall be provided FOB
Destination Within Consignee's Premises from/to the Richard L.
Roudebush VA Medical Center, Indianapolis, IN. There shall be no
additional charge for mileage and/or transportation incurred by the
Contractor. 4. It is impossible to determine the exact quantities that
will be required during the contract term. The Contractor shall be
required to provide all services that may be ordered during the term of
the contract (except as otherwise stated herein). The fact that all
quantities are not ordered shall not constitute a price adjustment to
the contract. 5. All general linen shall be in bundles of 25,
cross-stacked. 6. VA will furnish the transport containers. 7. Surgical
packs and wrappers/linens listed by type and usage: (NOTE: On all packs
with usage described as "As Requested", the average annual usage is 15
each; all quantities are listed as "each") PACKS: Aortagram -- As
Requested; Basic Drape -- 70; Basic Gown -- As Requested; Cardio Drape
-- 40; Cysto -- 24; Chest Sheet -- As Requested;EP -- As Requested;
Endo Pack -- As Requested; ICU Pack -- As Requested; Prep Sheet -- 40;
Single Gown -- 45; Towel -- 95; Thyroid -- As Requested; Thyroid Sheet
-- As Requested; Laporotomy -- As Requested; WRAPPERS/LINENS: 54" x 54"
wrapper -- As Requested; 45" x 45" wrapper -- 125; 36" x 36" wrapper --
125; 24" x 24" wrapper -- 125; 10" x 10" wrapper -- 30; Gown -- 75;
Towels -- 800; Pillow Cases -- 100. 8. Surgical hand towels shall be
quarter-folded. 9. Surgical scrubs shall be hung by size. 10.
Contractor shall furnish all supplies (i.e., sterilizer test strips,
sterilizer tape, etc.). 11. For the purposes of this contract, "patient
wearing apparel" and "uniforms" are not the same. 12. All linen repairs
will be made by VA. 13. Shirts, pants and lab coats are the "uniforms"
that shall be processed under this contract. All uniforms shall be
hung by ID number or by size per the following: Shirts (ID number),
Pants (ID number), Lab Coats (size). 14. Indianapolis VA Standards are
as follows: (a) Receipt/Release schedules must be met and shall be
based on a schedule to be established by the Contractor. One (1)
occurrence or failure to meet the established schedule per month will
be allowed. Compliance with standard will be determined by daily VA
review of receipt and release services, (b) Soiled/Clean Weight Equal
for Month -- Soiled weight shall equal exchange cart clean weight. The
allowable deviation from this standard will be 10% of the gross weight
for the month. Compliance with standard will be determined by VA review
of delivery ticket records for the month, linen inventory control
procedures, daily weighing of soiled and clean items, and weekly
reconciliation of totals, (c) Clean Laundry -- Laundry must be clean,
dry, free of lint and odor, and spots and stains must be removed. A
deviation of .25% per month will be allowed. Compliance with standard
will be determined by a daily random sampling by VA of clean linen on
a cart, (d) Whiteness and Tensile Retention -- Test specimen shall be
laundered twenty (20) times in accordance with standard textile
laboratory practice. A 5% deviation per test (after washing) for
whiteness and tensile strength loss will be allowed. Compliance with
the standard will be based on Contractor-furnished test specimens from
an independent laboratory, (e) Proper Textile Finish for Items -- All
articles shall be properly processed and shall be free of wrinkles and
scorches. A deviation of 2.5% per month will be allowed. Compliance
with the standard will be determined by a daily random sampling by VA
of clean linen on a cart, (f) Cross-Stacked Items in Counts of 25 --
Items shall be cross- stacked and piece counts shall be accurate. A
deviation of 2.5% per month will be allowed. Compliance with the
standard will be determined by a daily random sampling of clean linen
on a cart, (g) Damaged Clean Linen Items -- Laundered items shall not
be physically damaged due to improper processing or carelessness. A
deviation of .25% per month will be allowed. Compliance with the
standard will be determined by a daily random sampling of clean linen
on a cart, (h) Provide Data -- Data/information required herein shall
be submitted in a timely manner in accordance with the requirements of
this contract, (i) Quality Control Program -- the Contractor shall
institute and maintain a Quality Control/Quality Assurance Program
(QAP) to ensure that contract requirements are met. VA will review the
procedures in the program on a periodic basis to ensure adherence to
the program's contents, (j) Publications Maintenance -- All current
publications applicable to standard commercial practices, procedures
and processes within the industry shall be posted at the Contractor's
facilities and made available for VA review, (k) Clean
Laundry/Transport Carts -- All laundry/linen transport carts shall be
cleaned before being loaded with clean linen. A deviation of 2.5% per
month will be allowed. Compliance with this standard will be determined
by random VA sampling, (l) New Employee Training and Developmental
Training -- The Contractor shall maintain standard operating procedures
and records with respect to the training of new employees and
developmental training provided to established employees. These
procedures and records shall be made available for VA review. Award
