65 -- HOME OXYGEN SERVICE SOL RFP 521-025-98 DUE 102897 POC Teresa Posey, Administrative Contracting Officer, (205) 933-4492 Provide home delivery of oxygen to beneficiaries of the eight (8) Department of Veterans Affairs Medical Centers (DVAMC) located in VISN 7. The VISN 7 DVAMC's are as follows: Birmingham, AL; Central Alabama Veterans Health Care Systems (Montgomery, AL and Tuskegee, AL); Tuscaloosa, AL; Decatur, GA; Dublin, GA; Augusta, GA; Charleston, SC and Columbia, SC. Period of service will be from January 1, 1998 through September 30, 1998, with four (4) one (1) year renewal options. The contractor must be JCAHO certified. The contractor will be required to set-up all new prescriptions within 24 hours of initial notification. Response time to patient calls when there is an equipment or oxygen supply problem shall be within 24 hours. The 24 hour a day 7 day a week response time is inclusive of weekends and holdidays. The contract will be awarded one of the following two (2) ways: 1. Award for each State. 2. Entire VISN Award. Proposals submitted will be evaluated in descending order of importance. A) Quality Assurance Plan, B) Past Performance and Demonstrated Capability, C) Personnel Qualifications and D) Price.
