synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6 as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. Solicitation number N00211-97-Q-0015 applies and
is issued as a Request for Quote. The Naval Hospital, Materials
Management Dept., Purchase Division, Great Lakes intends to acquire
through other than full and open competition, in accordance with FAR
6.302-1, a quantity of 720 kits of Test Pack Plus HCG from Abbott
Laboratories, Diagnostics Division, Abbott Park, IL in support of the
Naval Hospital, Laboratory Dept., Great Lakes, IL. The requirement is
for a firm fixed- price purchase order for the following line item to
be provided for one (1) base year and three (3) one-year option
periods. Lot I is the Base Period which spans from 01 Oct 97 through 30
Sep 98 and includes Line Item 0001. Line Item 0001 is the 3A60-16 Test
Pack plus HCG for pregnancy testing,packaged as kits. The kits must be
usable with urine or serum; must require only the placement of the
sample into the kit (no other reqgents used in test procedures); must
be sensitive to 25 mlU/ml of HCG concentrations. Lot II -- First Option
Year - 01 Oct 98 through 30 Sep 99 includes Line Item 0002 which is
identical to Line Item 0001 above. Lot III -- Second Option Year -- 01
Oct 99 through 30 Sep 00 includes Line Item 0003 which is identical to
Line Item 0001. Lot IV -- Third Option Year - 01 Oct 00 through 30 Sep
01 includes Line Item 0004 which is identical to Line Item 0001.
Delivery: Delivery shall be made to the Naval Hospital, Bldg 81-H,
Great Lakes, IL 60088-5255. 30 kits to be delivered on the first and
fifteenth of each month. Regular warehouse receiving hours are between
7:30 a.m. and 4:00 p.m., Monday through Friday. All proposals shall
specify FOB destination. Options: FAR 52.217-9 "Option to Extend the
Term of the Contract" is hereby incorporated in full text into subject
solicitation. (a) The Government may extend the term of this contract
by written notice to the Contractor within 30 days of contract
expiration, provided, that the Government shall give the Contractor a
preliminary written notice of its intent to extend at least 60 days
before the contract expires. The preliminary notice does not commit the
Government to an extension. (b) If the Government exercises this
option, the extended contract shall be considered to include this
option provision. (c) The total duration of this contract, including
the exercise of any option under this clause, shall not exceed four (4)
years. This solicitation and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 90-46. The

paragraph (b), the following FAR clauses are incorporated by reference:

following DFAR provisions and clauses apply to this solicitation and
are incorporated by reference: DFAR 252.212-7000 Offeror Representation
and Certificaiton -- Commercial Items DFAR 252.212-7001 Contracting
Terms and Conditions Required to implement Status Applications to
Defense Acquisitions of Commercial Items. A proposal from the
aforementioned sole source provider in response to the solicitation
must be submitted in accordance with the instructions set forth in FAR
52.212-1 paragraph (b). As a minimum, the proposal shall include a
price proposal on company letterhead or the SF 1449 for the requested
line items showing unit price and extended price. If the SF 1449 is not
used, the proposal must include a statement specifying the extend of
Agreement with all terms, conditions and provisions of solicitation.
The proposal must also include technical information, remittance
address, completed copies of FAR 52.212-3 and DFAR 252.212-7000 and any
warranty, price or discount terms. Copies of clauses 52.212-3 and
252.212-7000 will be provided upon offeror request. Responses to this
solicitation are due no later than 3:00 p.m. CST, 24 Sep 97. All offers
shall be sent to Naval Hospital, Materials Management Dept., Purchase
Division, Bldg 81-H, Great Lakes, IL 60088 and should reference

capability to respond to the requirement or to submit proposals. This
notice of intent is not a request for competitive proposals. A
determination by the Government not to open the requirement to
competition based upon responses to this notice is solely within the
discretion of the Government. Information received as a result of this
notice of intent will normally be considered soley for the purposes of
determining whether to conduct competitive procurement. The applicable
