combined synopsis/solicitation for the purchase of a commercial item.
This notice is prepared in accordance with the format in Subpart 12.6,
as supplemented with additional information included in this notice.
The announcement constitutes the only solicitation and is issued as a
Request for Proposals. A written solicitation will not be issued. This
solicitation document and incorporated provisions are those in effect
through Federal Acquisition Circular 90-46. This VA Medical Center
anticipates the award of a contract and subsequent delivery order for
twenty six each Blood Fluid and IV Fluid Warming Unit, ten cases of
applicable Disposable IV Fluid Warming Sets for use with the proposed
system, and twelve each operators/maintenance manual for the proposed
system. The following minimum salient characteristics must be met for
consideration of a proposal: (I) Mechanical -- Mount for attachment to
I.V. poles with varying diameters This is a combined

I.V. poles with varying diameters and lightweight to permit ease of
carrying, moving and IV pole attachment; (II) Fluid Flow Path -- a.)
Flow rate range 75 to 5,000 cc/hr, b.) Disposable single patient use
heat exchange/tubing set that may be removed and reinstalled while
connected and in use without breaking sterile path, c.) Luer lock
tubing connectors to prevent accidental disconnects and provide
compatibility with hand pumps and infusion sets, d.) Long patient
delivery tubing to minimize additional extension sets, e.) Minimal
priming volume not to exceed 20cc; (III) Temperature Regulation and
Alarms -- a.) Digital temperature indicator with 0.1 degree centigrade
increments and back-lighting or illumination in darkened rooms, b.)
Outlet temperature to be no less than 34 degrees centigrade over full
range of delivery, c.) High temperature visual and audible alarms and
high temperature safety limit to prevent fluid temperature from
exceeding 42 degrees centigrade; (IV) Electrical -- a.) 120 Volt AC
line powered, b.) Must operate on existing AC hospital power with no
additional power conditioners, c.) Power cord must be at least eight
feet in length with AC line plug; (V) Conformance to Standards -- a.)
All current standards (AAMI, ANSI, ASTM, AABB, and FDA) for fluid
warmers must be met that are in effect at the time of equipment
requisition. The proposal shall include FOB destination delivery and a
one-year equipment warranty for parts and labor and the availability
of an extended warranty plus loaner availability. Further, service
support to include literature, parts, special test fixtures and
training must be available for the VA Biomedical Engineering
Department. Delivery date shall 30 days from the date of receipt of an
awarded delivery order. Federal Acquisition Regulation (FAR)
provisions at 52.212-1, 52.212-2, and 52.212-3 apply to receipt of
offers and are hereby incorporated by reference. Further, FAR clauses
52.212-4 and 52.212-5, as applicable, will apply to an awarded contract
and are also included in this notice by reference. FAR clause
52.227-19, "Commercial Computer Software—Restricted Rights and
52.249-8, Default are added as addenda and are hereby incorporated by
reference. These provisions and clauses are available on the Internet
Web at http://www.arnet.gov/far or may be obtained from the Contracting
Officer by faxed request through (214) 372-7066. The Government will
award a contract resulting from this solicitation to the responsible
offeror whose offer conforming to the solicitation will be most
advantageous to the Government, price and other factors considered.
Evaluation of offers will be made based upon price, technical features,
and past performance all of which will be given equal value and
considered through use of the best value methodology. Price evaluation
shall not only include the cost of the equipment but shall also
include the cost of disposables to be offered at a firm fixed price for
a period of one year. Technical evaluation will include comparison of
the proposal to determine if salient characteristics have been met,
reliability of the proposed equipmentand accessories and any technical
feature that will enhance the minimum salient characteristics given
above. Past performance will include offeror's experience in provision
of the requested or similar product through Federal Supply Schedule
(FSS) contract or open market, warranty terms and compliance therewith,
and timeliness of delivery. Offerors are reminded to address all
evaluation factors in their submittal. Interested parties are
encouraged to submit offers along with technical descriptive
literature, FSS contract information if applicable, and FAR 52.212-3,
"Offeror Representations and Certifications." Only written offers, sent
to the attention of the Contracting Officer, will be accepted.
Facsimile of proposals is acceptable however, It shall be the offeror's
responsibility to assume the risk of disclosure and to confirm receipt
of the transmission. In any event, all offers must be received prior
