SOL RFQ 796-014 DUE 092397 POC Mick Dubuis 813-972-7593 Fax 813-903-4838 This is a combined synopsis for commercial items prepared in accordance with subpart in 12.6, as supplemented with additional information included in this notice. This announcement and a list of pre-existing furniture and add-on furniture that the government still requires, will constitute the only solicitation; quotations are being requested and a full written solicitation will not be issued. To obtain the previous mentioned list, potential offerors are encouraged to contact the point of contact listed in this notice. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-01. Under the authority in FAR 6.302-1 (ii) The James A. Haley VA Hospital intends to negotiate a sole source procurement with Milcare, a Herman Miller company for add-on hospital furniture. This furniture is to be of hospital grade and be in accordance with strict infectious control standards. All furniture must be consistent with hospital appearance and 100% compatible with the existing Milcare furniture. Furniture is to be delivered to Vandenberg Air Force Base, located at 30 MDG/SGSL, 338 S. Dakota Ave., Building 13850, Vandenberg AFB, CA 93437. If submitting a quote on a brand name equal, adequate technical information, especially the quality assurance documents for infectious control, shall be submitted with the quote for the government's complete evaluation. Failure to submit adequate technical information shall be grounds for rejection of quotations. Adequate technical information shall be include but not be limited to technical specifications, product availability, product picture, product features, etc. Quotations shall be submitted on an "all or none basis." Partial quotations shall be rejected. All quotations shall state delivery time, F.O.B., F.O.B. point, shipping weight and cost if any. Offerors shall provide a complete set of provisions with quotation found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply. Quotations shall be submitted to the address listed herein or faxed to 813-903-4838 and directed to the attention of Mick Dubuis (90C). The government shall not be held responsible for quality of fax transmittals. The solicitation number (RFQ number) shall be listed on the outside of your response for identification purposes. (0251)
