of Work (SOW) is corrected from "...15,00 military and civilian
personnel..." to "...15,000 military and civilian personnel..." (ii)
This will be a firm fixed price contract. (iii) The due date of
proposals is unchanged. (iv) No further questions will be accepted
after the date of this amendment. (v) Paragraph C.3.1., Customized
referrals, is hereby renumberred to C.3.1.1. (vi) Paragraph C.5.,
Deliverables second items is hereby shortened to read
"Marketing/Promotional materials." Quantity for this item is changed to
"market up to 15,000 as appropriate." Delivery date for the development
remains 30 calendar days after contract award. (vii) The folowing is
added to the SOW as replacement page for pages 8 and 9 -- Paragraph
C.6. Evaluation Criteria: The Government will award the contract to the
Offeror whose offer represents the best value to the Government on the
basis of the merits of the offer, past related experience, and the
offeror's capability. The proposal will be evaluated according to the
following criteria, qualification of personnel, experience and past
performance, sample of deliverables, and price. A. Technical Criteria
1. Personnel Qualifications- Offeror must demonstrate personnel
qualifications: a. Administration/Program Manager (key personnel): The
contractor shall include a resume for key project manager assigned to
perform management functions under this contract. The resumes shall
include a detailed explanation of education, experience, training, and
recent projects. Experience must show responsibility for supervision
of counselors in a program directly for government personnel. b.
Contractor must demonstrate qualified counselors: Counselors shall have
completed formal education to the level of at least a Bachelor's
Degree, augmented by a minimum of one year's experience in the field of
child care; early childhood and elementary education; school age
programs; special education; elder care, gerontology; nursing;
rehabilitativetherapy; social work; or human services and counseling.
c. Ability to provide a timely response to DLA requests for information
and assistance as evidenced by written customer service standards and
staff performance expectations. 2. Experience and Past Performance: a.
Offeror must demonstrate, through examples of past contract efforts
(awarded within the preceding three (3) years); in-depth knowledge and
experience with at least three (3) organizations of comparable size,
mission and geographic distribution. Include the name, address,
telephone number, and facsimile (FAX) number of contact person. b. The
contractor shall provide specific examples of successful marketing
efforts designed to increase customer use of services offered. c.
Demonstrates ability to provide information and assistance specific to
various geographic locations including knowledge of local or State
laws and regulations pertinent to the topical area's cited in section
C.3. SOW. 3.Samples of Deliverables: The offeror shall submit no more
than two samples each of the following: a. Copies of actual information
kits, guidebooks, checklists, seminar handouts and other relevant
materials which are currently available and will be provided to DCMC
personnel. b. Samples of Executive Summary, Annual Reports, and Trend
Analysis of the type which would be submitted to DCMC official to
identify issues of particular concern, including usage rate and
recommend strategies for enhancing its use, and recommends strategies
for better tailoring the services to DCMC employee's needs. c. Samples
of a marketing plan and types of materials to be used to orient
employees to the resource and referral services. B. Price. Technical
Factors #1 Personnel Qualifications and #2 Experience and Past
Performance weigh equally; #3 Samples of Deliverables weighs less than
either #1 and #2. Total Technical factors weigh more than Price.
However, as proposals become more technically equal, price becomes more
important. The following are provided: Q: Section C.5. -- Deliverables,
(2) Marketing/promotional materials (15,000) copies addressed to each
employee to be ready 30 calendar days after delivery by US Postal
Service, who pays postage? If combination of inter-office mail and US
Postal Service, what percent by each and how will DLA identify? A:
Please refer to paragraph C.3.8. of the SOW. Q: Can you expand upon
C.3.3. which says, "The contractor shall provide access to vendor's
information and resources via the Internet." A: Please refer to the
SOW. Q: If there have been other vendors who have asked questions,
could I get copies of those questions and answers? A: All questions and
answers are provided to all interested parties via an automated
amendent to the solicitation. Q: Please specify the data the Government
will provide to the winning contractor after contract award, for
example, publications on parenting/education and Eldercare, and
electronic data such as referral data and DLA benefit programs. A:
Please refer to the SOW. Q: Will the Government provide the 1-800
numbercarrier, such as the FTS-2000 or FTS 2001 VAN systems? A: Please
refer to paragraph C.2. of the SOW. Q: Is the travel required in this
project for the eight (8) seminars only, or will there by other travel
requirements. A: Please refer to paragraphs C.3.5. and C.3.9. of the
SOW. Q: Does the Government anticipate the contractor to provide the
Live Telephone Counselors services for 24 hours a day, or during
business hours (e.g. 8:00 AM to 8:00 PM eastern time) and an Automated
Response system during off-business hours? A: Please refer to
paragraph C.2. of the SOW. Q: Please spell out and clarify DCMDI. A:
Please refer to paragraph C.l. of the SOW. Q: Please explain why DLA
employees need to know the referral's legal, tax and payroll
information, and how the contractor is to obtain this information. A
Please refer to paragraph C.3.1. of the SOW. Q: Please explain with
what the contractor is to be familiar in regards to the Tricare
Regions? A: Please refer to paragraph C.3. of the SOW. Q: Please
explain what thecontractor must do for inventory and distribution of
the information materials: for example, development, reproduction
and/or searching/collecting the materials from Government Agencies and
private sector organizations. A: Please refer to paragraph C.3.3.of
