This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. Proposals are being requested and a
written solicitation will not be issued. This solicitation no. RFQ
546-3-98 is issued as a Request For Quotations. Incorporated provisions
and clauses are those in effect through Federal Acquisition Circular 46
and VAAR 96-28. The Standard Industrial Code (SIC) is 7629 and the
small business size standard is $3.5 million. Contract will be firm,
fixed-price. Requirement is to provide to the VA Medical Center, Miami,
Florida, FOB destination, all material, labor, supplies, parts, and
supervision to perform quarterly inspection, calibration, preventive
maintenance and unlimited repair service for 20 hemodialysis machines
by Fresenius USA. Response time is not to exceed 24 hours after receipt
of telephone notification by the VA. Quarterly maintenance service to
include but not be limited to: (1) Internal clean-out (2) Inspection of
all components for wear, adjustment, alignment, lubrication, and
operation to the manufacturer's published specifications as of that
date. (3) Repair or replace defective components to bring the equipment
to the manufacturer's specifications. (4) provide maintenance safety
inspection. Contractor will supply written procedures to meet JCAHO
documentation requirements as set forth in "management of the
environment of care EC.2.13. After each service Contractor is to apply
a gummed label, dated and signed, certifying performance and safety to
meet the manufacturer's published specifications as of that date and
provide a certification worksheet on each machine tested. Contractor
will contact the biomedical instrumentation service (Gil Wagener), Room
B116, phone 305/324-3183 to arrange for service dates. Contractor will
sign the biomedical log upon entering and leaving the Medical Center.
Service tickets will be signed by VA Biomedical instrumentation
personnel. Payment will be made quarterly after receipt by the VA of
service tickets, and certification worksheets and properly prepared
invoice. Contractor will provide certification of equipment maintenance
training for each service engineer assigned to this contract. Periods
between scheduled maintenance calls shall not be less than 90 days to
begin in October 1997. Contract period is to begin on 10-1-97 or at the
expiration of the warranty period listed for each piece of equipment
and to end on 9-30-98 with options to award for each of two additional
one year periods. Contract service is required for the entire period
on (8) eight Model No. 2008E machines and (7) seven Model No. A2008
machines; service beginning at expiration of warranty period on January
8, 1998 on (1) one Model No. 2208H machine and (4) four Model 2008H
machines. Offers will be considered only from offerors who are
regularly established in the business called for and who in the
judgment of the Contracting Officer are financially responsible and
able to show evidence of their reliability, ability, experience, and
that their facilities and the personnel directly employed or supervised
by them are able to render prompt and satisfactory services. Interested
firms should submit information or literature to support these
capabilities with their quotations. All work shall be performed during
the normal working hours of 8:00AM to 5:00 PM, Monday through Friday
excluding Federal holidays. Contract is subject to the availability of
fiscal year 1998 funding. The following provisions and clauses shall

or Executive Orders-Commercial Items apply to this request. Pursuant to
52.212-2 The Government intends to make a single award to the
responsible offeror whose offer is the most advantageous to the
Government. Award will be based on (1)technical acceptability as
determined by the literature submitted which demonstrates ability to
perform required functions. (2)past performance and (3)price. The
following clauses and or provisions are added by addendum: VAAR
852.270-4 Commercial Advertising; FAR 52.217-9 Option To Extend the
Term of the Contract, FAR 52.222-48 Exemption from Application of SCA
Provisions, FAR 52.237-2 Protection of Government Buildings, Equipment
and Vegetation, VAAR 852.270-1 Representatives of the Contracting
Officer. Quotations shall include all information required in 52.212-1,
a completed copy of provision 52.212-3, and information required by
52.222-48. Quotations must be submitted to the attention of Jane
Ferencz(90C) at the VAMC 1201 NW 16th St., Miami, FL 33125. All offers
must be received not later than close of business (4:00 PM E.S.T.) on
9-22-97. Late offers will NOT be accepted. Discussion will be held
