(i) This is a combined synopsis/solicitation for quote for commercial items & services prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Request for Quote (RFQ); quotes are being requested & a written RFQ will not be issued. (ii) The RFQ Number is DAHA04-97-T-9439. (iii) The RFQ and incorporated provisions & clauses are those in effect through Federal Acquisition Circular 90-46. (iv) This RFQ is 100% set-aside for small business concerns and/or Federal Supply Schedule contractors. SIC Code: 1752, Size Standard is $7.0M. (v) One line item. Carpet removal & Replacement in Bldg. 119 is considered one job, lump sum. (vi) Services, non-personal : Contractor to provide all labor, materials, equipment, transportation, other items & services necessary to remove & replace carpet throughout Bldg 119, Channel Islands AGS, Port Hueneme, CA. The Contractor shall remove & dispose, off-base existing glued down carpet, to include scraping of excess old glue, carpet backing residue, etc. The contractor shall purchase an estimated 1130 SY of carpet tiles. The contractor shall install an estimated 1530 SY of carpet tiles in corridors and offices. Carpet tiles shall conform to the following Brand Name specifications. Manufacturer: Bentley Mills, Manufacturer cannot be substituted due to previous purchase of approximately 400 SY of carpet tile. Style: Wallasey Loop carpet tile. Model: WL30B-6205, Color: Crosshills. Size of Tile: 18" X 18". Contractor shall provide nontoxic adhesive for carpet that adheres to the manufacturers specifictions for this type of installation. Approximate QTY: 9 pails. All seams & joints shall be properly sealed. Contractor shall purchase & install new vinyl 4" base cove throughout the building. Base cove shall conform to the following brand name specifications: Brand: Johnsonite, Color: Black Rubber, Qty: 2,640 lineal feet. Contractor shall be responsible for removal & disposal, off-base, of all cuttings & scrap pieces from the work site. The contractor shall be responsible for the removal & replacement of all systems furniture that is presently on the carpeted areas. (vii) Contractor shall contact the designated Government Point of Contact at least 14 calendar days prior to starting work to coordinate scheduling and access to the work site. Contractor shall specify the number of days required to complete the above outlined work in the submitted quote. (viii) Federal Acquisition Regulation (FAR) provision 52.212-1, Instructions of Offerors-Commercial Items applies to this acquisition. (ix) FAR provision 52.212-2, Evaluation-Commercial Items, appliesand quotes shall be evaluated for fair and reasonable price, technical acceptability, adherence to applicable clauses and provisions and satisfactory performance record.
