This is a combined synopsis/solicitation for Commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote (RFQ) is F6550197QP311. The provisions and clauses incorporated are those in Federal Acquisition Circular 90-46, and it has a standard industrial code(SIC) of: 3523. Line item 0001: Shredder Operations: Qty sixty (60), Unit of issue: Days, Price per day with extended total. Option 1: Price per day for shredder operation over 60 days. Period of Performance will be 1 Oct 97 through 30 Sept 98. Wage Determination No. 94-2017 Rev #8 Applies. FOB Point Elmendorf AFB Alaska. The services required by this SOW include contractor furnished labor, supervision, transportation, tools, equipment and minor materials necessary to accomplish the Operation of Maxigrind Shredder. Elmendorf AFB (EAFB) has recently installed a MAXIGRIND E425 commerical shredder, hereafter referred to as the shredder, to reduce wood debris and tires to material that can be utilized in base operations. This will allow the base to send less bulk material to the local landfill. The shredder will be operated as necessary on an intermittant basis. This statement of work is to obtain contractor services to operate the shredder on an as needed basis. The operation will include loading stockpiled materials into the shredder, the shredding of bulk materials to a rolloff dumpster, and keeping the shredder yard clean. It is mandatory that two people be present at all times during shredder operation. The contractor will be responsible for all periodic and preventative maintenance. The contractor personnel shall have a thorough working knowledge of the shredder operation. Training by the manufacturer, or a suitable substitute, and a manufacturers operating manual will be provided. The contractor shall maintain a qualified work force. The contractor shall be responsible for operations associated with the shredder. The contractor shall report within five days of being notified by the designated representative. The contractor shall load the materials into the shredder with a small front end loader, operate the shredder, and place ground materials into the rolloff dumpster. The rolloff dumpster will be located in the shredder yard. The contractor shall contact the designated representative when operations at the shredder yard are complete. The contractor shall be responsible for keeping the shredder yard clean and free of hazards. The contractor shall be responsible for performing all necessary preventative and periodic maintenance to the shredder as specified by the manufacture. The contractor shall immediately contact the designated representative when the shredder is found to be in need of repair. Emergency repairs $2,500.00 and less will be paid for via IMPAC card. All repairs of $2,500.01 and up will be separately priced, negotiated and awarded. The contractor shall insure the project activities are conducted in a manner consistent with all applicable USA, OSHA, state and local health safety requirements. The contractor shall provide all labor, materials, tools, and equipment necessary to complete the tasks described in this statement of work with the exceptions of those items specified in the technical exhibits. The Air Force will provide the contractor access to all available documents necessary to the accomplishment of the requirements of this statement of work including an operations manual. All material, equipment and facilities listed in the technical exhibits, 5a and 5b, will only be used for the purposes specified in the contract. The contractor shall be held liable for damage caused to the equipment and facilities through improper operation or negligence. The contractor shall immediately notify the Contracting Officer and the designated representative or alternate via telephone of conditions noted or observed during this effort which indicated a potential imminent health or safety risk. A written notification detailing the nature and extent of the potential risk and the supporting documentation shall be delivered to the designated representative within three days after the telephonic notification. The contractor shall notify the Contracting Officer and the TPM or alternate within twenty four hours of any contact with representatives of any regulatory agencies.TECHNICAL EXHIBIT 5a, Government furnished facilities. The facility is located at building 33-395 on Elmendorf and the building is heated, has a restroom, water, and electricity. The facility has two lockable gates and a fence around the perimeter. TECHNICAL EXHIBIT 5b, Government furnished Equipment. The following pieces of equipment will be supplied for contractor usage: 1 fully operational Rexworks E425 Maxigrind commercial shredder. 1 fully functional Bobcat 753 front end loader. 31 Cubic yard rolloff dumpsters as needed. The following provisions apply 52.212-1, Instructions to Offerors, Commercial Applies to this acquisition and any addenda 52.212-3, Advising Offerors to include a completed copy of representation and certifications-commercial items with offer 52-212-4, Contract terms and Conditions-Commercial items, applies to and a statement regarding any addenda to clause, and 52.212-5, Contract terms and conditions required to implement statues or executive orders- Commercial items, applies to this acquisition.
