Semi-annual visits will occur in March and August, 1998, on all equipment, except the Philips R/F System and related equipment, which will occur in January and July, 1998. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement reconstitutes the only solicitation; Requests for Quote (RFQ) are being requested and a written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. This procurement is unrestricted. SIC: 8999. Contractor shall provide semi-annual inspections and preventive maintenance, consisting of technical inspections, cleaning, oiling, adjusting, calibrating, and maintaining the equipment in first class operating condition, to the VA Northern Indiana Health Care System, Fort Wayne Campus and Marion Campus. The contractor shall make thorough technical inspection of the equipment in accordance with the commercial practice governing maintenance of the type of equipment involved. Offerors are requested to provide their most current catalogue price list, the monthly cost for services, and information to support past performance requirements. This will be a firm fixed-price contract for the period October 1, 1997 through September 30, 1998. FOB is destination. Payment will be made semi-annually, in arrears, on certified invoices.
