Scheck, (216) 838-6088 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation 541-201-97 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. The VA Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106 intends to negotiate sole source with Parks Medical Electronics, Inc., P.O. Box 5669, Aloha, OR 97006 for procurement of Parks Medical Electronics Flo-Lab, Model 2100, which shall consist of the following components: one (1) Pentium Tower Computer, 16MB RAM, 1.6 GB Hard Disk,33.6K Modem, 8X CD-ROM; Win95 Operating System and PC Anywhere for remote diagnostics; one (1) SVGA color monitor, one (1) Hewlett Packard Deskjet 870CXI color printer; VIP software with special features for Model 2100 only; Two frequency, bi-directional Dopplers (1-4 MHz and 1-8 MHz); System required by the VA shall meet the following requirements: System provides forward and reverse doppler flow indicators; dual channel PPG (Photoplethysmography); dual channel PVR (Pneumoplethysmography); automatic cuff inflator; remote bedside control for cuff inflation, doppler volume control with mute, navigation arrows, auto scale, and data storage; single/dual channel chart recorder; mobile cabinet with two drawers and storage space; complete cuff package for all arterial and venous studies; "Start up" supply package; fully automated computer integrated multi-port cuff inflator and spectrum analyzer. Two (2) days on-site training is also provided. Due to the unique and complex nature of the procurement, there is a reasonable basis to conclude that the supplies/services required are only available from this source with unique capabilities, and no other type of supplies or services will satisfy agency requirements. Delivery will be within forty-five days after award. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition with the following changes, items (e) and (h) of the standard clause are deleted. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offer. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.203-6, 52.203-10, 52.219-8, 52.219-14, 52.222-26, 52.222-35, 52.222-36, 52.222-37 and 52.225-3. The following VAAR clauses are incorporated and apply to this acquisition 852.210-70, 852.210-71 (which provides for guarantee of equipment in accordance with standard industrial practice in effect at time of award) and 852.219-70, Veteran-Owned Small Business. This notice is not a request for competitive proposals. However, all proposals received within the due date for receipt of proposals will be considered by the Government. A Determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Interested offerors may submit their proposals along with a completed copy of the provision at 52.212-3 Offeror Representations and Certifications-Commercial Items, on or before 4:30p.m., September 26, 1997.
