66 -- 48 CHANNEL CARDIOLAB INTEGRATED ELECTROPHYSIOLOGY SYSTEM SOL 558-89-97 DUE 092697 POC Amelia Evans (919) 286-6914 Ext. 6445 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 558-89-97 applies and is issued as a request for proposal and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 97-01. The Standard Industrial Classification Code is 5047, and the business size standard is 100 employees. Contractor shall provide: (Item #1) 48 Channel CardioLab System with CardioEP Electrophysiology Software consisting of the following: CLabPro-600 System Controller (1 ea.) to include 180 MHz Pentium processor, 64 MB of RAM memory, 3.5", 1.44MB floppy disk drive, and High Performance Optical Drive II System; Amplifier Unit to include superior RF filtering and 64 catheter input capability with 2 catheter input modules, 12 Lead ECG, 4 pressure inputs, and 50' fiber optic cable; Monitors and Printer to include two (2) 21" flat screen high resolution (1600 x 1280) color monitors, HP LaserJet 5 with 600dpi capability, 12 page/min. and full page graphics, and 2MB of memory; Software to include current version of CardioEP Electrophysiology Software and modem communication for instant support; Accessories to include workstation mobile cart with swivel arms, 60'(153cm)W X 34"(86cm)H X 30"(77cm)D, 33.3K Baud modem, spare 50' fiber optic cable, spare set of 2 catheter input modules, four (4) 650 MB optical disk catridges, isolation safety transformers and uninterruptible power source, and required accessories and manuals; training and inservice; one (1) year warranty on all hardware including parts and labor, software maintenance updates and telephone support; (Item #2) RF Ablation Device Interface Module (1 ea.) to include necessary hardware and software for recording and displaying real-time information from the following RF ablation units: EP Technologies, Inc. Delivery: FOB Destination within 60 days of contract award.
