The District of Columbia, Department of Health Services through the ADP Acquisitions Branch, Program Support Center, U.S. Department of Health and Human Services intends to issue a Request for Proposal (RFP) to acquire homeless support team services for individuals who are homeless and mentally ill in the District of Columbia. The Contractor shall provide all necessary personnel, supplies, materials for Intensive Case Management inclusive of long term support for approximately 150 clients (and an expected increase in capacity of up to 250 clients during the first option year). The clients shall be provided with comprehensive continuum of care to assist them in becoming empowered and support in moving from homelessness to living and working in the community. The anticipated period of performance is a basic period of one year with four (12 month) option periods. RFP 213-98-1003 will be issued approximately 15 days from the date of publication of this notice. Multiple contract awards are anticipated.
