65 -- SEQUENTIAL COMPRESSION DEVICE, SCRUB BRUSH, HYPODERMIC NEEDLE SOL SP0200-97-Q-4221 POC Buyer: Mickie Smith/215-737-2296 or KO: Michael Schmitt/215-737-2665 The Defense Personnel Support Center intends to issue a Request For Quotation (RFQ) for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), Subpart 12.6, as supplemented with additional information included in this notice. The written RFQ will be issued on or about October 15, 1997. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 90-46. The RFQ will be issued under Simplified Acquisition Procedures (SAP) under the Test Program (FAR 13.6). A potential of three indefinite delivery, indefinite quantity contracts are anticipated to be awarded as a result of the solicitation. Each awarded contract may be up to a $5 million maximum amount. Each contract will include a base year and possibly two option year periods. The items are listed as follows: Sequential Compression Device, estimated minimum annual expenditure $400,000. Surgical Scrub Brush, estimated minimum annual expenditure $150,000. Needle, Hypodermic, 18 gauge x 1 1/2, estimated minimum annual expenditure $100,000. Actual quantities or dollar expenditures anticipated will be spelled out in the RFQ. The evaluation of quotations and the determination as to equality of the product shall be the responsibility only of the Government and will be based on information furnished by the vendor. Requests for the solicitation packet must be in writing and be received before RFQ documents will be mailed. While price is a significant factor in the evaluation of offers, the final award decision will be based upon a combination of price, delivery and past performance as described in the solicitation. This request will be issued vial a written RFQ, the estimated award date will be November 7, 1997. All responsible sources may submit an offer which DPSC shall consider. The purchasing activity is not responsible for locating or securing any information which is not identified in the quotation and reasonably available to the purchasing activity. If the vendor proposes to modify a product so as to make it conform to the requirements of this announcement, he shall include a clear description of such proposed modification and clearly mark any descriptive material to show the modifications.
