Lewis (504) 568-0811, extension 3254 Contractor shall provide on-site supervision and personnel on a daily basis to cover the operations of a copy center for patient records. Contractor will be required to process all requests for patient information received from, but not limited to, insurance compainies, physicians, patients, attorney's, state and federal agencies. Contractor shall have directly or indirectly (2) years of experience in copying medical documents/records. Contractor shall also have on staff at least (1) credentialed medical record professionl. Work to be performed at the VAMC, N.O. Solicitations will be issued on or about October 17, 1997. Request for documents must be in writing and can request can be faxed to (504) 589-5289. This procurement is a turnkey operation and is being done in accordance with FAR Part 12, Commerical Items. Bid opening will be held on Wednesday Novermber 19, 1997 at 2:00 p.m. at the VAMC, bldg 2, 4th floor, Room 409, 1601 Perdido Street, New Orleans, LA.
