A -- STUDY OF DURATION OF LABOR AND CESAREAN DELIVERY IN ASSOCIATION WITH EPIDURAL ANALGSIA SOL NOCHD-DESPR-97-SS1 POC Mya N. The National Institute of Child Health and Human Development (NICHD) plans to enter an Interagency Agreement with the Department of Defense, Military Hospital, The Tripler Army Medical Center in Honolulu, Hawaii, using other than full and open competition to conduct the study of Duration of labor and cesarean delivery in association with epidural analgesia in nullipara women. The determination to use other than full and open competition was made in accordance with 41 USC 253 (c)(1) and is based upon the following factors: 1) The Tripler Army Medical Center in Honolulu, Hawaii is a large tertiary hospital and the only medical facility in the military base, serving the military personnel and their family. 2) There are about 2,700 deliveries per year. 3) The patient population is well defined with mixed race/ethnicity (Asian, Blach and White).4) The use of epidural analgesia was rare. The proposed period of performance of the study is one year. (See Numbered Note 22). Qualification of Hospitals for the Epidural Analgesia Study must have the following: 1) The hospital must have at least 2,500 deliveries a year; 2) The hospital must be the only maternity care provider in the region; 3) The obstetric population must be well defined with homogeneous socioeconomic status; 4) Patients must have universal access to prenatal care; 5) Decisions on major obstetric procedures must not be influenced by socioeconomic status; 6) The hospital must have a sufficient number of patients if Asian, Black and White ethnicity; 7) Use of epidural analgesia must be rare until three to four years ago; 8) Use of epidural analgesia increased rapidly and dramatically in the last three years; and 9) Change in the obstetric population and attending obstetricians should be minimal in the last five years. The purpose of this synopsis is to determine whether there are sources with the above qualification to perform the work described above.
