This is a combined synopsis/Request for Quotation in accordance with FAR 13, Simplified Acquisition Procedures as supplemented with additional information. This announcement constitutes the only request for quotation. Proposals are being requested and a written request for quotation will not be issued. This synopsis/request for quotation incorporates provisions and clauses in effect through Federal Acquisition Regulation Circular 90-46. The Standard Industrial Classification Code is 8011. This procurement is issued as a total 100% set-aside small business. The Contractor shall provide Emergency Room (ER) physician service to the Tuba City Indian Medical Center, Tuba City, Arizona. The ER physician shall provide emergency clinical services inclusive of medical and surgical care for the entire spectrum of diseases and injuriesof all ages within the limit of defined clinical privileges. The ER physician shall provide direct services, including assessment, diagnosis, emergency treatment of serious life threatening conditions. The Contractor shall comply with all requirements, policies, and procedures of the service unit. The Contractor shall provide on-site ER physician services from 8:00 PM to 8:00 AM, Friday through Sunday including Federally recognized holidays. The Contractor to provide an estimated 864 hours. The anticipated award is from date of award through September 30, 1998. The Contractor shall be under the administrative supervision of the ER Department Supervisor and clinical care will be monitored by the ER Department's Quality Assurance Plan. The Contractor is subject to character investigation as required by P.L. 101-630, the Indian Health Child Protection and Family Violence Prevention Act prior to performance of a Contract. The Contractor is covered by the Federal Tort Claims Act during the delivery of health careservices in a facility owned, operated, or constructed under the jurisdiction of IHS. The services provided must have been within the Statement of Work. The Contractor shall be credentialed and Clinical privileges shall be granted by the Clinical Director at Tuba City Indian Medical Center prior to providing services. Qualifications: (1) board eligible or board certified in emergency medicine or board certification in pediatric, internal medicine or family practice with one year's experience or equivalency in emergency care; (2) Valid current full and unrestricted license to practice medicine, in State, district of Columbia, the Commonwealth of Puerto or a territory of the United States; (3) Current certification in PALS,ACLS, and ATLS; (4) Three letters of reference are required to verify current competency within last year and satisfactory interpersonal working relationships; (5) Offeror's past and current professional experience dealing with Native Americans and/or experience in providing services on a Reservation or in a remote location; (6) Health status allowing physical and mental ability to carry out required functions, verified by the employing agency or furnish a copy of the physical examination. Technical proposals will receive paramount consideration in the selection of award, however, not to the exclusion of cost and price consideration. The response to this request for quotation shall be submitted in two parts: response to technical evaluation criteria(s) and a price quote for the service.
