This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number 5114-8-3070 is issued as a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-01. The standard industrial classification is 2721 and the business size standard is 500 employees. This is an unrestricted procurement. There are no addenda to these provisions. The provision at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (AUG 1996) applies to this acquisition. The vendor responding to this acquisition should provide the following: A price quote on company letterhead forthe requested items, stating unit price, extended price, F.O.B., prompt payment terms, remittance address, past performance information to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information), and a completed copy of FAR clause 52.212-3, Offeror Representations and Certifications Commercial Items (JAN 1997). Provision FAR 52.212-2(a), Evaluation Commercial Items (OCT 1995) is included and states: "The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate offers: quality of service, past performance on similar contracts, and price, with quality and past performance being first in importance."
