The Albuquerque Area Indian Health Service will be requesting for proposals for Periodontic Dental Services from interested providers. The acquisition is released as a Total Small Business Set-Aside. Contractor must have graduated from an approved Periodontal Specialty Program. Services to be provided to IHS beneficiaries who are provided services at the Acoma-Canoncito-Laguna Service Unit. All proposals will be accepted and considered by the Government. A fixed price contract will be awarded for a base year and three, one-year options for renewal. Contractor to provide no more than 36, 8-hour clinic days per contract year. Prospective contractors must possess a current and unrestricted license to practice dentistry in any one of the 50 states within the United States, and must possess current medical malpractice insurance. Projected release date of the Request for Proposal is scheduled on or about October 22, 1997 and will close on November 21, 1997. Projected award date of contracts is December 3, 1997.
