Berry, Contracting Officer (334/727-0550, X-3513) Provide pharmaceutical services in the Dothan, Alabama area to VA beneficiaries as prescribed by the health care provider at the Dothan VA Community Based Outpatient Clinic located in the Dothan, Alabama area. The Dothan VA Community Based Outpatient Clinic is a new facility without a history of quantities required; therefore estimated quantities cannot be projected. The Contractor shall be required to fill and dispense all prescriptions of up to ten (10) day supply of medication as ordered. The Pharmacy is to be located in the Dothan, Alabama area. To enhance patient compliance, the contractor will be required to provide teaching counseling to patients and family members regarding the intended use of the medication, possible side effects and drug interactions considering health and disease status. The following factors will be used to evaluate offers: Personnel Qualifications, Pharmacy Management, Past Performance, Location and Cost. Bid opening is tentatively scheduled for November 26, 1997.
