U -- CASE MANAGEMENT SOL RFP 335-1-98 DUE 120397 POC Janice Byrd 612-725-2188 Provide Case Management Services for eligible veterans enrolled in the Department of Veterans Affairs Regional Office and Insurance Center Vocational Rehabilitation and Counseling Division program, authorized by the VA Regional Office, Bishop Henry Whipple Federal Building, One Federal Drive, St. Paul, MN 55111. The services will be provided on an as-needed basis in accordance with the terms and conditions of the solicitation. These services are provided for eligible veterans with compensable service-connected disabilities who are in receipt of services under 38 U.S.C., Chapter 1, Part 21. Case Management Services include, 38 U.S.C. Chapter 31 support and supervision. As part of this Case Management a contract counselor will provide face-to-face supervisions; prepare documentation for purchase by VA of books, tools, supplies; evaluate need for medical, psychological and dental services, respond to phone inquiries, provide problem solving counseling (separate from normal case management) and complete necessary narratives and documentation. Proposals will be accepted from eligible firms regularly established in the business. Offeror must be physically located in the area where services are required. The VA may award one, multiple or no contracts in various areas, as it deems in the best interests of the Government. Contract(s) will be awarded for the period of January 1, 1998 through December 31, 1998 with four one-year options to renew, subject to availability of funds. A pre-bid conference is scheduled for November 17, 1997. Bid documents will be available on or about November 3, 1997 with Bid Opening on or about December 3, 1997.
