Belcher, Administrative Contracting Officer, (205) 933-4492 The DVAMC, Birmingham, AL requires full on site service maintenance, repair services and preventive mainenance for a Baxter Automatic Tablet Counter ATC 212 System, Model: Optiplex 433 DXE, for the period of 1-1-98 through 9-30-98, with four (4) one year option renewal options. The contractor will be required to perform preventive maintenance (PMI's) semi-annually. These PMI's shall be within the first five (5) working days of the months of January and April (Option year months shall be: October and April). All maintenance is to be in accordance with the manufacturer's instructions and must meet all NFPA and JCAHO requirements. Service calls will be 24 hours per day 7 days per week, 365 days per year. Service response time of four (4) hours from notification and on-site within twenty four (24) hours will be required. Specific reporting requirements will be required and will be strictly enforced.
