Administration (SSA) intends to issue a sole-source purchase order to
Anacomp, Inc. of Silver Spring, Maryland for the acquisition of
Datagraphix Laser Xp silver microfilm, 16mm x 200m, catalog number
E021436, (20 rolls/case) and Datagraphic Laser Xp silver microfilm,
105mm x 200m, catalog number E021596, (2 rolls/case). Anacomp, Inc. is
the only known source which can provide the required film. This film
is market proprietary and is not available through dealers or
distributors. This acquisition will be conducted under the authority of
the Test Program For Commercial Items contained in the
FederalAcquisition Regulations (FAR) subpart 13.6 and as authorized by
41 U.S.C. 253. Specifically FAR subpart 13.601 (a) provides
authorization to utilize simplified procedures for the acquisition of
commercial items which do not exceed $5,000,000,including options. This
is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this solicitation; quotations are
being requested and a written solicitation will not be issued. This is
a simplified acquisition issued as a request for quotation (RFQ),
number SSA-RFQ-98-3415. This request for quotation and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-01. REQUIREMENT: One hundred fifty three(153) cases, (20
rolls per case), of Datagraphix Laser Xp silver microfilm, 16mm x 200m,
catalog number E021436 and three hundred thirty (330) cases of
Datagraphix Laser Xp silver microfilm, 105mm x 200m, catalog number
E021596, (2 rolls/case). The Government intends to award a firm
fixed-price purchase order. Delivery shall be made to the Social
Security Administration, Attn: Receiving Clerk, 300 N. Greene Street.,
Baltimore, Maryland 21201. Offeror is also requested to supply FOB
Point, (city, state), and the weight of each case of film, 16mm
and105mm. The provisions at FAR Part 52.212-1, "Instructions to
Offerors -- Commercial Items", and FAR Part 52.212-2, "Evaluation --
Commercial Items" apply to this acquisition. There are no addendums to
these provisions. To be considered for award under this solicitation,
offeror must offer a firm, fixed unit and total price for the required
line items. EVALUATION CRITERIA: The offer will be evaluated to assess
compliance with the specifications set forth in this solicitation and
to determine the reasonableness of the offered pricing. AWARD IN
AGGREGATE: The award of any contract as a result of this solicitation
will be made in the aggregate. Offer must quote on all items in the
request for quotation. Failure to quote on any item shall be cause for
rejection of the offer as being nonresponsive. The total evaluated
price will be based on the unit prices for each item multiplied by the
quantity. All the individual totals will be added to obtain a grand
total. A written notice of award or acceptance of an offer, mailed or
otherwise furnished to the successful offeror within the time for
acceptance specified in the quote, shall result in a binding purchase
order without further action by either party. Offeror is advised to
include a completed copy of the provision at FAR Part 52.212-3 Offeror
Representations and Certification -- Commercial Items, with their
quote. The provisions at FAR Part 52.212-4, Contract Terms and
Conditions -- Commercial Items apply to this acquisition, with an
addendum to incorporate the following clauses by reference: 52.203-3
Gratuities, (Apr 84); 52.225-11 Restrictions on Certain Foreign
Purchases, (Oct 96); 52.232-17 Interest (Jun 96); 52.232-33, Mandatory
Information For Electronic Fund Transfer Payment, (Aug 96). The
following provisions at FAR Part 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Executive Orders )
Commercial Items apply to this acquisition: (a) (1), (a) (2), and (b)
(1) through (3), and (6) through (9) and (11). Offers in response to
this RFQ shall be submitted by November 04, 1997, 3:00 P.M. Eastern
Standard Time, addressed to the Social Security Administration, Office
