synopsis/solicitation for a commercial item prepared in accordance with
the format in Subpart 12.6, as supplemented with additional information
included in this notice. This announcement constitutes the only
solicitation; proposals are being requested and a written solicitation
will not be issued. Solicitation No. F08650-97-TP-939 is issued as a
Request for Quotation. The solicitation document and incorporated
provisions and clauses are those in effect thru Federal Acquisition
Circular 90-46 and thru Defense Acquisition Circular 91-12. This
solicitation is 100% set-aside for small business. It is advertised
with a Standard Industrial Classification Code (SIC) of 7991, with a
size standard of $5 million. Contractor shall furnish all labor,
material, music, transportation, and all other items necessary to
provide low impact, high intensity and step aerobics on the the
following schedule: Patrick Fitness Center, Bldg 546, Step Class,
Monday thru Friday, 0600, 1100, 1645, and Saturday at0930; Patrick
Fitness Center, Bldg 546, Low Impact, Monday thru Friday, 0900; Patrick
Fitness Center, Bldg 546, Circuit Training, Monday thru Friday, 1200;
and Cape Canaveral Fitness Center, Bldg 49915, Step Class, 1130 and
1700. Instructors shall possess a valid National Aerobics Instructor
and CPR Certification which shall be provided prior to commencement of
work. Class will be a minimum of 45 minutes in duration and limited to
a maximum of 60 students per instructor. Class shedule may be adjusted
based on participation for which a 24 hour notice shall be provided by
the government. Cancellations will only be acceptable on emergency
basis with a 12-hour cancellation notice. The contractor will not be
required to perform classes on Federal Holidays which are; Columbus
Day, Veterans Day, Thanksgiving Day, Christmas Day, New Years Day,
Martin Luther King Jr. Day, Presidents Day, Memorial Day, July 4th, and
Labor Day. This contract will be for one base year and four option
years. (Line Item 0001) Est Quantity—1,040 Ea; Basic performance
period 1 Dec 97 through 30 Sep 98; (Line Item 0002) Est. Quantity --
1,248 Ea; Option Year, performance period 1 Oct 98 through 30 Sep 99;
(Line Item 0003) Quantity -- 1,248 Ea; Option Year II, performance
period 1 Oct 99 through 30 Sep 00; (Line Item 0004) Quantity -- 1,248
Ea; Option Year III, performance period 1 Oct 00 through 30 Sep 01;
(Line Item 0005) Quantity -- 1,248 Ea; Option Year IV, performance
period 1 Oct 01 through 30 Sep 02. The following provisions and clauses
apply to this acquisition: FAR 52.212-1 Instructions to Offerors --
Commercial Items is hereby included by reference. The following are
addenda to FAR clause 52.212-1; At paragraph (c), Period of Acceptance
for Offers, the amount of time an offeror must hold its prices firm is
hereby changed. An offeror must hold the prices in its proposal, firm
for 60 calendar days from the date specified for receipt of proposals.
Paragraph (e) multiple offers has been determined by the Contracting
Officer to be non-acceptable to this solicitation and is hereby removed
in its entirety for the purpose of this requirement. Offerors are
advised that proposals made be evaluated without discussions. Award
might be made to the responsible offeror whose proposal is responsive
to the terms of the RFQ and is most advantageous to the government. In
accordance with FAR subpart 9.104-1, each offeror is required to
provide information on a reasonable number of recent and relevant
contracts for the same or similar items, as well as the offeror's
primary financial institution, to include contract numbers,
points-of-contacts, telephone numbers, and any other relevant
information, which will be used by the Contracting Officer to make a
responsibility determination about a prospective offeror. Far provision
52.212-2 Evaluation -- Commercial Items applies to this solicitation
and the following evaluation criteria (arranged in their relative order
of importance) are hereby incorporated: Award will be made to the
offeror who meets the solicitation's minimum criteria for technical
acceptability at the lowest price. To be determined technically
acceptable, the offeror must furnish a valid National Aerobics
Instructor and CPR Certification which demonstrates that the services
offered meet every requirement stated in the purchase descriptions
given above for Items 0001 -- 0005, Professional Qualifications, and
past performance. FAR provision 52.212-3 Offeror Representations and
Certifications -- Commerical Items shall be completed and submitted
