This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. A) Solicitation F41612-98-R0008 is being issued as a request for proposal. B) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-01. C) This is 100% set aside for small business, SIC 5047, size standard 500. D) Line 0001-2EA ultrasound training simulator, must contain mainframe with CPU, control panel, display screen, simulator transducer, full-size mannequin, portable mannequin table. Must include software for training on following types of ultrasound exams: gyn, ob, abdominal, endo-vaginal, vascular, trans-rectal and superficial structures. E) Must include four copies of operation literature and two copies of service literature which must contain all circuit theory, schematic diagrams, inter- connecting diagrams, all maintenance and calibration procedures, circuit card component layouts and identification and illustrated parts breakdown. F) Delivery required 30 days ARO; delivery to be made to Sheppard AFB TX, FOB destination. G) Provisions at FAR 52.212-2, Evaluation-Commercial Items is incorporated by reference with the evaluation to be included in para (a) of that provision to be 1.-price, 2.-past performance.
