268-4239 Contractor will be responsible for the recycling of
non-harzardous materials and treatment of mercury and any other
dazardous materials in lamps for proper disposal. Process to ensure
separation of glass, metals, and hazardous materials. Contractor to
provide container for storage of lamps and ballasts. Prices will
include all costs, including processing costs, labor, transportation
and state fees. Contractor to provide the Industrial Hygiene Office at
310-268-3368 at the VA Project Engineering office with exact count per
type of lamp and ballasts, prior to removing from the facility and as
witnessed and signed by the Industrial Hygienist and Project Engineer.
Contractor will provide copies of all necessary permit (s) names and
locations of treatment and disposal facilities and their permits, from
cradle to grave, prior to pick-up. Contractor to issue evidence of
recycling/disposal following each pick up to Industrial Hygiene Office
(130B) Bldg. 220, Room 207A and Project Engineer (138E) Bldg. 218,
Room 315. Industrial Hygienist will sign all manifests.Contractor will
not exceed maximum amount per line item. Firms desiring consideration
