This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal RFQ 98-01 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-01. The Standard Industrial Classification Code is 5047 and the business size standard is 100 employees. The James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, FL 33612, is seeking a contractor to provide on a brand name or equal basis, the following equipment. NOTE: ALL THE FOLLOWING EQUIPMENT IS RELATED AND INTERLOCKS. All equipment is for Elmendorf AFB, in Anchorage AK and all pricing shall be destination. Three Each -- AMSCO 3085 Major Surgical Table: For remote-controlled, flexible, articulated posturing of the surgical patient, return-to-level tabletop movement. Electric with 2" foam tabletop pad set, 120 volt. (This table has a 500lb weight capacity). Six Each -- IV ARMBOARD WITH 2" DAD PAD: Supports arm at a comfortable angle. Locks by gravity to table side rail. Adjusts 180 degrees horizontally. Supplied with a 15-inch Velcro arm restraint strap. Velcro tape allows pad to be quickly attached to and/or removed from board. Compatible with OrthoVision, Ortho 2 1080, 2080, and 3080, Surgical Tables. Six Each -- conductive restraint strap: Designed with quick-release buckles on both ends for easy, positive adjustment. Extra-wide webbing is soil-resistant, washable and electrically conductive. Wide section of strap measures 6 inches. Length adjusts to fit all body types. Compatible with 1080, 2080, and 3080 Surgical Tables. Six Each-CONDUCTIVE RESTRAINT STRAP: Designed with quick-release buckles on both ends for easy, positive adjustment. Three Pair -- BEIRHOFF KNEE AND FOOT REST ASSEMBLY: Contoured to provide comfortable leg support for patient in lithotomy position. Fully adjustable so that legs can be elevated to facilitate positioning. Includes straps. Requires Clark Sockets or Tri-Clamps. Compatible with 1080, 2080, and 3080 tables. One Each -- URO/ENDO ACCESSORY PACKAGE FOR 3080 AND 3085 SURGICAL TABLES. Includes: Foot control, Drain pan, Patient transfer board with 2" pad, Uro X-ray Top without Pad, IA Extender with 2" pad and x-ray top, Cable Wrap. Three Each -- EYE-ENT-NEURO HEADREST ADAPTER: This specialized adapter accommodates all standard eye-ENT-neuro attachments. Conveniently inserts into either end of the 3080 table. Compatible with 3080 table. Three Each -- FOOT EXTENSION WITH LOCKS: Extends table to accommodate taller patients. Swings 90 degrees upward for foot support when patient is in reverse Trendelenburg. Full-width extension locks to table foot section side rails. Includes side rail locks. Compatible with 3080 Surgical Tables. One Each -- CARDIOVASCULAR ACCESSORY PACKAGE FOR 3085 AND 3080 SURGICAL TABLES: Includes: Fem/Pop Board and Perineal Cutout Filler Piece. Proposals shall be submitted on an "all or none basis." Partial proposals shall be rejected. If submitting on a brand name equal basis, sufficient technical data must be included for the government's evaluation.
