99 -- STORAGE OF FILES SOL RFQ 546-04-98 DUE 111497 POC Point of Contact -- Laura Nasso,305-324-3290 STORAGE OF FILES RFQ 546-04-98 This is a combined synopsis/solicitation for commercial items/services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation ; proposals are being requested. Solicitation RFQ 546-04-98 is issued as a request for proposals and incorporates provisions and clauses that are in effect through Federal Acquisition Circular 46. The Standard Industrial Classification Code (SIC) is 6324. Contract shall provide monthly service for storage of records for the V.A. Medical Center, Miami. The period of contract will be from 12-1-97 to 9-30-98, with 2 year option to renew. Vendor shall provide storage for approximately 8,500 boxes, with increases or decreases at any given time throughout the agreement. Accessing of individual files or boxes from storage with delivery to designated area at VA Medical center and return to storage area when requested. Procurement of new boxes pick-up form designed room and added to existing computer inventory. Supply storage boxes for files. Monthly computerized inventory sheet with bar codes mailed to COTR. Destruction service by shredding, fax send/receive record information, release of record information via telephone to designated individual. Priority delivery with 1 hour from initial request for records. Termination charges to include pulling boxes from storage area, palletizing and placed on vendor dock for pick-up (if applicable).
