This is a combined synopsis/solicitation for commercial items (brand name or equal) is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information provided in this notice. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-45 and Defense Acquisition Circular 91-11. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. Solicitation number N62604-98-Q-7551 is issued as a Request for Quotation (RFQ). The standard classification code is 5047, small business size standard is 500 employees. A firm-fixed price purchase order will be issued under the Simplified Acquisition Procedures. Contract Line Item 0001 is for five (5) each Electromyograph, Laptop, Portable, -- performs EMG/Nerve conduction studies; must be able to perform automatic data calculations/auto- storage/auto-report generation; EMG and printer to be portable and lightweight (combined weight not to exceed 20 lbs), to allow for studies to be conducted in remote areas in the hospital and other regional medical facilities; unit to be able to interface with PC computer and use RISC Digital Processor with flash memory system; must be capable of single fiber EMG studies and some automatic analysis R-R' interval analysis; Excel Tech P/N 10048D or equal. Line Item 0002 -- five (5) each Portable Printers for the above system, Excel P/N 10051 or equal; Line Item 0003 -- five (5) each Carrying Cases for Electromyograph, Excel P/N 101018 or equal; Line Item 0004 -- five (5) each Carts with casters for electromyograph to include a printer shelf and accessories Basket, 3.5" casters, Excel P/N 10050 or equal; Line Item 0005 -- five (5) each arms for Medical Cart listed above, Exel P/N 10061 or equal; Line Item 0006 -- five (5) each cable and software, parallel to parallel, to allow data to be directly transferred to an external PC, in Flat ASCII Format forpermanent digital storage and/or database requirements, Excel P/N 10054 or equal; Line Item 0007 -- five (5) each accessory Kits for electromyograph to include: 2 each finger clip electrodoes w/24" leads, 1 each bar electrode w/9mm concave disc & 24" lead, an actively-shielded 3 lead electrode cable with mini-alligator clips for active, reference and ground connections, 100 tab type disposable pre-gelled adhesive electrodes for active and reference sites and 20 larger tap type disposable pre-gelled ground adhesive electrodes, 2.9mm tin disc electrodes with 24" leads, a ground electrode plate with 48" lead, and a bottle of Signa cream conductive electrode paste; Line Item 0008 -- one (1) job to provide on-site training for operators of the electromyograph at the Naval Medical Center, Portsmouth, VA. Two (2) Sets operating and service manuals; and on-site operator/user training for nursing and technical staff shall be furnished. Delivery FOB Destination, Gulfport, MS, 30 calendar days after award. This solicitation is issued as a request for quotes (RFQ). The Government reserves the right to make award of the initial offer received without discussions. The Government will award a contract resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation will be most advantageous to the Government.
