433-1112 (Site Code DADA16) This is a combined synopsis/solicitation
for a commercial service prepared in accordance with the format in FAR
Subpart 12.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation;
proposals are being requested and a written solicitation will not be
issued. The solicitation number is DADA16-98-T-0017 and is issued as a
request for quotation. The solicitation and incorporated provisions
and clauses are those in effect through Federal Acquisition Circular
90-45. This procurement is 100% set aside for Small Business. The
Government intends to negotiate solely with Hewlett Packard Company,
3000 Minuteman Rd MS 0400, Andover, MA 01810, for a firm fixed price
contract providing maintenance service for the period of December 1,
1997 through September 30, 1998 with an option to renew for four
additional one year periods. Tripler Army Medical Center intends to
contract for the following services: Line Item 0001 -- Base Year:
Furnish Labor/Parts/Material/Equipment to maintain in operating
condition the on site government owned HP Monitoring System in accord
with manufacturer specifications and special conditions herein, 10 mo.
Line Item 0002 -- Option Year I: Furnish
Labor/Parts/Material/Equipment to maintain in operating condition the
on site government owned HP Monitoring System in accord with
manufacturer specifications and special conditions herein, 12 mo. Line
Item 0003 -- Option II: Furnish Labor/Parts/Material/Equipment to
maintain in operating condition the on site government owned HP
Monitoring System in accord with manufacturer specifications and
special conditions herein, 12 mo. Line Item 0004 -- Option III: Furnish
Labor/Parts/Material/Equipment to maintain in operating condition the
on site government owned HP Monitoring System in accord with
manufacturer specifications and special conditions herein, 12 mo. Line
Item 0005 -- Option IV: Furnish Labor/Parts/Material/Equipment to
maintain in operating condition the on site government owned HP
Monitoring System in accord with manufacturer specifications and
special conditions herein, 12 mo. Include 4.167% General Excise Tax
when applicable. Must respond to service calls telephonically within
two (2) hours. Service calls will be performed within twenty four (24)
hours after notification between the hours of 8:00 a.m. to 5:00 p.m.,
Monday thru Friday, excluding Federal Holidays. All repairs requiring
replacement parts shall be completed within five (5) work days. Shall
provide a certified HP trained, qualified person to respond on-site.
Shall provide required HP factory trained technicians to assist
biomedical technicians over the phone, parts and for second response
repair as needed. Telephone support coverage will be 24 hours, seven
(7) days. If replacement parts are needed the contractor shall furnish
new or rebuilt manufactured replacement and/or repair parts, QA tested
and approved prior to installation. Contractor shall provide all parts
and necessary equipment to perform repairs and scheduled services. All
offers will be evaluated and considered in accordance with the
following evaluation factors: 1. Technical Experience 2. Past
Performance 3. Price The offeror will provide documentation of the
three evaluation factors. These factors are listed according to their
importance with the number (1) being the most important, then #2 and
#3. The following provisions and clauses apply to this acquisition.
