This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal (RFQ) 796-98-02 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-01. The Standard Industrial Classification Code is 3821 and the business size standard is 500 employees. The James A. Haley Veterans Hospital, located at 32000 Bruce B. Downs Blvd., Tampa, FL 33612 is intending to sole source to Steris Corporation of Mentor, OH for the procurement of six each -Model P1000 Scope Sterilizer Systems. This procurement is for Ft. Richardson, AK. Sterilizer must be a liquid chemical sterilizing system that can sterilize heat sensitive medical devices in 30 minutes or less. Sterilizer must be a portable type table top unit. Unit must be capable of sterilizing such products as: rubber, paper, plastic an lumen. Unit must have a FDA approved biological indicator. Unit must be FDA cleared for the sterilization of both flexible and rigid enodoscopes. Proposals shall be submitted on an "all or none basis." Partial proposals shall be rejected. Pricing shall be all inclusive. (F.O.B. Destination) Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
