H -- INSPECTION, EVALUATION AND ASSEMENT OF ENVIROMENTAL MANAGEMENT SOL IFB 667-06-98 DUE 121997 POC Dennis DeYong, 318-424-6084, Fax 318-424-6078 Contractor shall furnish all necessary on site supervision, labor, tools, equipment, transportation and material to provide inspection and monitoring services for the collection, evaluation, review, analysis and reporting environmental levels of glutaraldehyde, ethylene oxide, formaldehyde, nitrous oxide (waste anesthetic gases) and asbestos, at the Overton Brooks VAMC, 510 East Stoner Avenue, Shreveport, LA 71101. Services shall be provided in compliance with the State of Louisiana and Federal regulations, terms, conditions, provisions, schedules and specifications of this solicitation. Services are to be provided for a period of one (1) year from the date of award with the option to renew for four (4) additional one (1) year periods. Contractor qualifications: a. Contractor shall provide evidence of Louisiana State Accreditation. Certificates for each individual who will be performing on site, shall be submitted prior to award. b. Contractor personnel performing on site under this contract,at a minimum, shall be a professional industrial hygienists or industrial hygiene technician or a competent person/supervisor as defined in 29 CFR 1926.32 (f) and possesses the required accreditation as defined by the State of Louisiana Title 33, part III, chapter 27 & 51 and Federal Regulations. c. The laboratory shall be AIHA accredited and the microscopist must have been rated as "proficient" as determined by participation in the NIOSH PAT program.
