The Dept. of Veterans Affairs Medical Center, Louisville, Kentucky intends to procure on a competitive basis a firm fixed price,full service maintenance agreement on Government owned equipment as follows; 3M Steri Vac ETOH sterilizers model number 400BF, 400DGP, 487AGDP, Vintage premium #4 for sterilizer 400B, and ETOH monitor system utilizing ETOH detection badges for monitoring exposure levels. Service shall include two (2) preventive maintenance visits. Period of performance shall be from date of award to September 30, 1998 with three (3) one year options to renew.
