Provide Part Time dental hygienist services at the Coast Guard Clinic located in Port Angeles, WA. for active duty and retired military personnel and their dependents, referred for treatment by Coast Guard dentists. This will be a non-personal services contract, medical liability insurance coverage and license is required. Resulting contract will be accomplished by Simplified Acquisition Commercial Item methods. This contract is subject to the Service Contract Act. The Contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours per day, 16 hours per week. Period of performance will be 01 Jan 1998 through 30 Sep 1998 with one (1) option year. The RFQ will be issued on or about 15 Dec 1997 and closing on 30 Dec 1997. This procurement is a 100 per cent Small Business Set-Aside. The SIC code is 8021 and the small business size standard is $5 million average annual receipts for the preceding three (3) fiscal years.
