required for Fleet and Industrial Supply Center, Pearl Harbor. Locating
vendors who can provide counseling/referral services. Under the
Civilian Employee Assistance Program (CEAP), the contractor will
provide counseling/referral service (C/RS) to employees (and supporting
family members, as appropriate) of the following primary activities:
Navy Public Works Center, Pearl Harbor (approx. 1610 employees); Fleet
and Industrial Supply Center, Pearl Harbor (approx. 586 employees);
and the Naval Facilities Engineering command Pacific Division (approx.
567 employees); and the Naval Facilities Engineering Command Pacific
Division (approx. 567 employees). A request for assistance may be
received either directly from a Federal Civilian employee or through
the Federal employee's supervisor. In the case of a supervisory
referral, the Federal civilian employee's participation in counseling
beyond the initial interview is voluntary. When a participant is
involved in a crisis situation, the contractor will provide or arrange
for immediate assistance. Upon request for assistance, the contractor
will provide intake counseling consisting of the following: a. An
interview by a qualified counselor, b. Screening and diagnosis of
assistance requirements. c. Evaluation of the participant and
consultation, if necessary, with a specialist in the problem area; d.
Creation of confidential patient records; e. Obtaining employee
authorization to release information in the case of a supervisory
referral; f. Providing feedback on employee to the referring
supervisor; g. Explaining the need for and recommending, if necessary,
specialized assistance. After intake interviews are completed and a
participant's condition is diagnosed, specific referral to appropriated
counseling agencies will be made with the participant's concurrence.
the contractor will make specific appointments for care. The contractor
will follow-up with the employee to ensure quality of treatment and
with the agency to determine continuation of treatment. Whenever, an
employee is referred to a counseling agency, all cost of those services
will be borne by employee. The contractor will routinely follow-up each
referred case approximately eight weeks following the conclusion of
visit, in order to ensure that required counseling is initiated and
completed. Reports to supervisors, in cases of supervisory referral,
will be in person, in writing, or by telephone. The contractor will
again contact the employee six months from the date of service. This
allows time for resolution of issues to take place, yet offers
proactive opportunity to re- address unresolved or new concerns. When
requested, the contractor will provide guidance to supervisors on
proper way to approach an employee with a problem and for handling
complex and sensitive performance and personal problems. Sufficient
copies of written information in the form of a brochure will be
available from the contractor for distribution to all employees covered
by this contract. The written information will state the times and
telephones numbers at which participants may reach the CEAP counselors.
Posters or newsletters publicizing the program will be made available
for bulletin boards. Quarterly, the contractor will submit a report to
Human Resources Office, Commander Naval Base, Pearl Harbor, Hawaii
(HRO COMNAVBASEPH). Annually, the contractor will submit a summary of
the quarterly reports to the HRO COMNAVBASEPH. The report will contain
information for the current quarter and a cumulative record from the
beginning of the contract year. The contractor must agree to furnish
additional data, if any, for any CEAP reports required by other
agencies even if such data is requested after expiration of the
contract. The contractor will ensure that personnel assigned
responsibilities for direct contact with employees or supervisors,
either by telephone or during personal interviews, have the following
qualifications: a. At least a bachelor's degree in psychology,
counseling, social work of other directly related field os study; b.
Work experience with business, industry or government agencies in order
to assess probable causes of employee work performance problems; c.
Experience or knowledge in the area of alcoholism and other chemical
dependencies and co- dependencies; d. Knowledge of federal government
CEAP policies and procedures; e. Knowledgeable about community
resources. Contractor will provide resumes of personnel. The contractor
is responsible for obtaining and ensuring that the contractor's
employees obtain the necessary licenses, permits and insurance required
to perform the services covered by this contract. The contractor will
maintain separate records for each participant in accordance with the
Privacy Act of 1974. Except when otherwise required by law, the
confidential nature of these records will be rigidly observed by the
contractor. Information may be released whena participant authorizes
such releases in writing. Following expiration of the contract,
contractor is obligated to follow-up with appropriate treatment
resources and track participation of employees in the CEAP until
renewal of the contract or the transition to the new contractor is
completed (approximately 30 days). This obligation also requires the
submission of a corrected or supplemental quarterly report if
necessary. The contractor will be required to furnished all supplies
and materials to be used during the performance of this contract except
that specified below. Government will provide, on an as needed basis,
adequate office space, equipped with appropriate furnishings and
equipment for counseling clients. Keeping in mind that the primary
function of this service is the identification of problems, clinical
treatment and maintenance counseling are excluded from this contract.
The contractor will not refer participants to any source for which the
contractor receives payment or other consideration or in which the
contractor maintains a financial interest. Responsible sources who can
