DADA16) This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in Subpart 12.6, as supplemented
with additional information included in this notice. This announcement
constitutes the only solicitation to be issued. A written solicitation
will not be issued by the Government. Responses should reference
Request for Proposal No. DADA16-98-T-0056. This CBD solicitation
incorporates the provisions and clauses that are in effect through
Federal Acquisition Circular 90-45. This procurement is 100% set aside
for Small Business. Proposed contract is for a firm fixed price. The
Cardiology service shall provide clinical, diagnostic, and consultative
services in accordance with the terms and conditions stated in the
scope of work. The contractor will perform 15 procedures for the base
period and 17 procedures for three (3) option year periods. The base
period will begin on or about January 2, 1998 through September 30,
1998, with three (3) subsequent one year (12 mo) option year periods,
if exercised by the government. All offers will be evaluated and
considered on "Best Value" and price in accordance with the following
evaluation factors: (1) Technical Experience (2) Past Performance (3)
Price. Offerors shall submit verification of their Technical Experience
and Past Performance along with their proposal. These factors are
listed according to their importance with the number (1) being the most
important. The following provisions and clauses apply to this
acquisition. These clauses and provisions are available on the web at
http://farsite.hill.af.mil/. CONTRACT CLAUSES: Offerors are advised
