This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number N61339-98-Q-0155 is issued as a request for quotation (RFQ). All references to "offers" are understood to be "quotations." The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. This is a Firm Fixed Price requirement (FFP) and will be awarded on an "all or none" basis. The Standard Industrial Classification Code is 3577 and the small business size is 700. The Naval Air Warfare Center Training Systems Division (NAWCTSD), 12350 Research Parkway, Orlando, Fl 32826, intends to place an order with the Folsom Research,Inc., 526 East Bidwell St., Folsom, CA 95630 for the following item: CLIN 0001: VIDEO SCAN CONVERTER, MODEL 9400JR, QTY: 7 EA. The contractor shall deliver the entire quantity: (1) F.O.B. destination; (2) within 15 days of award; (3) ship to: ISEO, SUITE 368, 33150 NIXIE WAY, SAN DIEGO, CA 92147, ATTN: CHAU TRUONG. The following FAR/DFARs clauses and prescriptions apply and are incorporated by reference: FAR 52.212-1 "Instructions to Offerors Commercial Items (OCT 1995)" with the following addendum: Inspection and acceptance shall be made at destination; Provision 52.212-2 "Evaluation -- Commercial Items" with the following addenda: The Government intends to award a contract without discussion to the responsible offeror whose quotation conforms to the solicitation and provides the best value to the Government, price and other factors considered. The following factors will be used to evaluate quotations: (1) Past Performance, (2) Price, (3) Technical Capability. Evaluation of past performance shall be based on information provided by the offeror or through other sources (i.e., offeror's performance on previously awarded contracts with other Government or Commercial entities.) The offeror must identify at least two previous contracts for which the offeror has performed work similar to the statement of work in this solicitation. References should include the most recent contracts completed within the last two years. The offeror shall provide a point of contact, current telephone number and fax number for each contract. This list should be included with offer. Offers are to include a completed copy of the provision at 52-212-3, "Offeror Representations and Certifications Commercial Items (JAN 1997).
