Johns, (913) 682-8700, ext. 302 (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; quotations are being requested and written solicitations will not be issued. There will be a site visit conducted on December 18, 1997, at 1:00 p.m. at the U. S. Penitentiary, 1300 Metropolitan Avenue, Leavenworth, Kansas. It is not mandatory to attend the site visit, but it is highly recommended that you attend. (II) This solicitation is issued as Request for Quotation (RFQ) 132-0028-8. (III) This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 97-02. (IV) This solicitation is 100% set aside for small business. The Standard Industrial Classification Code (SIC) is 3699, with a size standard of 750 employees. (V) Line Item #1: Contractor to install two (2) electric gear-motors on rear sallyport gates #2 and #4. Line Item #2: Contractor to replace control panel located in base of tower #3 and control switches at officer station of tower #3. Line Item #3: Contractor to replace all required micro-switches on rear sallyport gates #1, #2, #3 and #4. Line Item #4: Contractor is to replace all wiring and related hardware from rear sallyport gates #1, #2, #3 and #4 to tower #3. NOTE: Contractors are hereby notified that security clearances are mandatory prior to award of this contract for all workers who will be on site. (VI) Delivery to be made no later than forty-five (45) days from receipt of order. Freight may be F. O. B. Origin, but Destination is preferred. Delivery address: U. S. Penitentiary, Attn: Receiving Depot, 1300 Metropolitan Avenue, Leavenworth, KS 66048. (VII) FAR provision 52.212-1, Instructions to Offerors-Commercial Items (Oct. 1995), is hereby included by reference. The following are addenda to FAR 52.212-1: At paragraph C, Period for Acceptance of Offers, offeror agrees to hold the prices it is offering for sixty (60) calendar days from the date specified for receipt of offers. At paragraph H, Multiple Awards, the Contracting Officer has determined that multiple awards are not applicable in this solicitation and is hereby removed in its entirety for purposes of this requirement. (VIII) FAR provision 52.212-3, Offeror Representation and Certifications-Commercial Items (June 1996), will be completed and submitted prior to award. (IX) FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items (Oct. 1995), hereby applies to this solicitation. The following is an addendum to FAR 52.212-4: At paragraph G, Invoice, contractor shall submit invoices to U. S. Penitentiary, Attn: Accounting Supervisor, 1300 Metropolitan Avenue, Leavenworth, KS 66048.
