Lancaster Rd., Dallas, TX 75216 has a requirement for one (1) Community Based Outpatient Clinic in each of the following counties of Texas: Cooke and Denton. Contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The Contractor shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceuticals, and all administration functions in accordance with the terms and conditions of the contract. Compensation will be made through a monthly capitation (per patient, per month) payment. The term of the contract shall be one (1) year with four (4) additional one-year option periods. Solicitation 549-30-98 will be isssued as a Request for proosal. Services will be procured in accordance with Federal Acquisition Regulation Part 12, Acquisition of Commercial Items. Issue date will be on or about 12/17/97. Proposal due date will be on or about 1/16/98.
