R -- MOCK SURVEY JCAHO SOL 541-107-98 DUE 010598 POC Rita Bonner, Contracting Officer (440) 838-6091 This is a combined spynopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request for Quotations (RFQ) are being requested and a written solicitation will not be issued. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Number 60. This procurement is set aside for small businesses and the SIC Code is 8742 and the small business size standard is $5.0 million. Services are required for a JCAHO pre-survey consultation utilizing the 1998 Joint Commission on Accreditation of Hospital Organizations (JCAHO) standards. Contractor shall provide all labor, materials and supervision to perform a JCAHO pre-survey during the months of February and/or March 1998, for the Veterans Integrated Service Network (VISN) 10 facilities at the following VA Medical Centers (VAMC) and Outpatient Clinic (VAOPC): VAMC Dayton, Cincinnati, Chillicothe, Cleveland and VAOPC Columbus,Ohio. The pre-survey shall address Medical Staff and Nursing and an assessment shall be made of each facilitites compliance with JCAHO standards. The following JCAHO priorities by facility should be used ot develop your offer: 1) VAMC Dayton -- HospitalAccreditation Program (HAP), Hospital Based Home Care (Durable Medical), Long Term Care (Sub-Acute) Dementia), Ambulatory, Domiciliary-Behavioral and Substance Abuse; 2) VAMC Cincinnati -- Hospital Accreditation Program, Long Term Care, Ambulatory, Home Care (Durable Medical Equipment), Domiciliary-Behavioral and Substance Abuse; 3) VAMC Chillicothe -- Hospital Accreditation Program, Ambulatory, Home Care (Durable Medical Equipment) Long Term Care and Behavioral; 4) VAMC Cleveland -- Hospital Accreditation Program, Ambulatory, Hospital Based Home Care (Durable Medical) Long Term Care, Behavioral and Substance Abuse; 5) VAOPC Columbus -- Ambulatory, Home Care (Durable Medical Equipment) and Substance Abuse. The pre-survey shall be performed through the use of interviews, document review, tour of patient care areas or other specific areas of the facilities, or other standard information gathering techniques. At the conclusion of the pre-survey, an exit conference shall be conducted to present a summary of findings and discuss areas of noncompliance and provide recommendations for compliance. A comprehensive written report outlining the findings and recommendations shall be provided to the VA within two weeks following completion of the survey. The actual dates of the pre-survey shall be coordinated with the Contracting Officer's Technical Representative. Services shall be performed Monday through Friday during the hours of 8:00 a.m. to 4:30 p.m. excluding Federal holidays. It is required that the Contractor and contractor personnel have actual JCAHO survey experience.
