Lancaster Road, Dallas TX 75216 has a requirement for one (1) Community Based Outpatient Clinic in each of the following Texas Counties: Eastland, Stephens, Palo Pinto, Parker and Hood. All five (5) counties are located west of Forth Worth, TX. The contractor shall provide for the delivery and management of continuous primary and preventive medical health care for all eligible Veteran Affairs enrolled patients. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, short-term pharmaceutical prescriptions and administrative support. Compensation will be made through a monthly capitation payment for each patient in accordance with the terms and conditions of this proposed contract.. The issue date will be 12/29/97, and the proposal due date will be 02/02/98.
