This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation document Request for Quotes (RFQ) No. 0024-8 and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation (FAR) Circular 97-02. This procurement is being advertised as a total set-aside for small business, under standard industrial classification code 8011 & 8049, with a small business size standard of 5.0 Million. Any quotes received from large business will not be considered for award. The Contractor(s) shall provide services for the following: Professional Dietician in a standard community Dietician practice to include, but not limited to, providing dietary examination evaluations of menu plans/special diets, recommendations for dietary care, and nutritional counseling. Professional Podiatry services, performance will include all duties expected of a Podiatrist in a standard community practice to include, but not limited to, examination, diagnosis, treatment, minor surgical procedures, when required. Professional Radiology services, performance will include, but not limited to, performing procedures such as barium swallows, upper g.i. series, intravenous cholangiography, intravenous pyelography & urograms and outpatient morbidity and procedures classification form as required. Provide professional Otorhinolaryngology (ENT Specialist) services, to include, but not limited to examination, diagnosis, treatment, and recommendations for care of various ear, nose, and throat conditions that require the attention of an ENT Specialist. To provide professional Urology services, but not limited to, examination, diagnosis, treatment, and recommendations for care of various urological conditions that require the attention of a Urologist. All contractors will present proof of education, training, and experience, and must be licensed, also maintain malpractice liability insurance. These are nonpersonal health care services in nature and they are for base year and four (4) option years to extend services. Correctional experience is desired but not mandatory for positions. Place of performance is the United States Penitentiary and the Federal Prison Camp located at: 3901 Klein Blvd., Lompoc, CA 93436. All responsible bidders will be considered.
