Jene (205) 933-4492 The DVAMC Birmingham, AL requires full service maintenance and repair services for a Prucka Engineering, 32 Channel Cardio Lab Computerized Eletrophysiology System and Cardio Lab Satellite System, for the period of 03-01-98 through 09-30-98, with four (4) one year renewal options. The Contractor will be required to perform preventative maintenance (PMI's) semi-annually. These PMI's shall be within the first five (5) working days of the month April for the initial period and within the first five (5) working days of the months of April and October for the each option year renewal. All maintenance is to be in accordance with the manufacturer's instructions and meet all NFPA and JCAHO requirements. Service calls will be five (5) days per week, between the hours of 8:00 am and 5:00 pm, Monday through Friday, Federal Holidays included. Service response time of four (4) hours from notification and be on site within eight (8) hours. Specific reporting requirements will be required and will be strictly enforced.
