services for the disposal of infectious medical waste at the Hill Air
Force Base Hospital located in Ogden Utah. Services include providing
containers, ranging in various sizes (10,20,44 gallon etc) and pick-up
of medical waste twice weekly. Contractor will have all applicable
permits of the handling, transportation, storage, treatment and
disposal of infectious waste. Contractor shall provide computerized
tracking and manifesting of all infectious waste services and an
activity statement that includes waste production by day, volume,
containers used and types of waste generated. Contractor shall
incinerate all waste picked up and all medical waste containers shall
carry the Department of Transportation Regulated Medical Waste
Transportation Number. On request contractor shall provide in service
training and 24 hour notice emergency services response when required.
Basic contract with 4 option years. The approximate issue/response
date will be 26 Jan 98. The anticipated award date will be 27 Feb 98.
Written procedure will be used for this solicitation. No telephone
requests. Only written or faxed requests received directly from the
requestor are acceptable. All responsible sources solicited may submit
a bid, proposal, or quotation which shall be considered. Request for
copies of solicitations must reference the entire solicitation number,
including purchasing office identification number. To expedite your
request, limit the number of individual solicitation numbers requested
per letter to not more than 10. Don't request information on this
requirement until after the issue date. An Ombudsman has been appointed
to hear concerns from offerors or potential offerors during the
proposal development phase of this acquisition. The purpose of the
Ombudsman is not to diminish the authority of the program director or
contracting officer, but to communicate contractor, concerns, issues,
disagreements, and recommendations to the appropriate government
personnel. When requested, the Ombudsman will maintain strict
confidentiality as to the source of the concern. The Ombudsman does not
participate in the evaluation of proposals or in the source selection
